HBV是乙型肝炎病毒的英文缩写，DNA为脱氧核糖核酸的英文缩写，HBV-DNA即乙肝病毒脱氧核糖核酸。 HBV是嗜肝DNA病毒。乙肝病毒脱氧核糖核酸（HBV-DNA）是指HBV的基因组，也就是含有HBV全部遗传信息的成分。它是由3200个“碱基对”组成，为环状部分双股DNA，分长的负链（L）和短的正链(S)。负链上有4个开放读码区（S区、C区、P区、X区）。 S区又分前S1、前S2和S基因，分别编码乙肝病毒包膜上的前S1、前S2蛋白和HBsAg，三者合称为大分子蛋白，前S2蛋白和HBsAg称为中蛋白，HBsAg为主蛋白。C区编码HbcAg，前C区与HBeAg有关。P区编码DNA多聚酶（DNAP）。X区编码X抗原（HBxAg）。 HBVDNA位于病毒的核心，与HBeAg几乎同时出现在血液中，为游离型HBVDNA，是乙肝病毒感染最直接、最特异和敏感的指标。在慢性感染的患者中，HBVDNA可以和肝细胞的基因整合，称为整合型HBVDNA。 HBVDNA的检测方法有定性和定量两种。前者一般用斑点杂交法检测，它的原理是利用有标记的特异性探针与被检测样本中的HBVDNA结合，再通过显色显示检测结果。用该方法可检测到0.2~1pg的DNA，具有简便、快速、经济等特点，但不能进行定量检测，灵敏度不很理想。 定量检测现多用荧光定量PCR，它通过用特异的引物与被检测样本中的HBVDNA结合。在一定条件下，在体外对HBVDNA进行扩增，同时它还设计标记有荧光的探针，该探针也能与样本中的HBVDNA结合，并每扩增一次就放出一个荧光信号，通过信号接受器、电脑程序处理后便可得出样本中HBVDNA的含量（拷贝数）。 定量检测方法与定性检测方法相比，其灵敏度明显提高，可检测到100~1000拷贝数的DNA（一般HBVDNA>1000copies/ml为阳性）。但它的费用高，需一定的实验设备和条件，且有时会因污染而得出假阳性结果。 HBVDNA定量检测在临床上具有十分重要的作用和使用价值。通过分析检测结果，我们可以了解病毒的复制状况。例如，HBVDNA>1000copies/ml或阳性，提示病毒有复制，而且拷贝数的高低与病毒复制的程度成正相关；若结果阴性，则表明病毒处于不活跃阶段，病情比较平稳。 医生可以参考HBVDNA的检测结果，来决定是否需要给患者进行抗病毒治疗。如果患者转氨酶升高（特别是转氨酶高于正常值两倍以上者），HBVDNA>1000copies/ml或阳性，则应予抗病毒治疗；对一些肝脏有进行性损害（经肝穿活检证实）、HBVDNA阳性的患者，即使肝功能正常，必要时也应考虑抗病毒治疗。 HBVDNA还可作为判定抗病毒药物疗效的指标之一。如果抗病毒治疗3个月或半年，HBVDNA仍不转阴或下降少于2次方，则提示该药物抗病毒疗效欠佳，可考虑停药或换用其他抗病毒药。
