Results The results showed that the levels of TNF-α in SF from OA were significantly elevated.Marked fibrosis and thickened intima were found in OA synovium, with much dense collagen,which,in some cases, was banded,but no specific inflammtory changes were detected.Type B cells in OA synovium had the characteristics of organelles of cells active in synthesizing and secreting protein.Type A cells in OA synovium had the features of the macrophage. Conclusion Evident fibrosis in synovium may be either mediated by the cartilage degradation product or by the enchanced joint damage process in OA. 【Key words】 Osteoarthritis Synovial fluid Tumor necrosis factor Microscopy,elec- tron 近年来，在关节疾病发生、发展机理的研究中，细胞因子的介导作用日益受到重视〔1,2〕。本研究通过检测及观察比较不同形式关节内损伤和骨关节炎（OA）滑液中肿瘤坏死因子(TNF-α)含量和滑膜超微结构的变化，旨在进一步探讨滑膜参与OA病理进程的机理。资料和方法 1.滑液标本：本组24例患者，其中OA行全髋、全膝关节置换术者9例（男5例、女4例，年龄62±4岁）；股骨颈囊内骨折行股骨头置换术者6例(男2例、女4例，年龄67±3.6岁)；股骨头无菌性坏死行股骨头置换术者5例(男4例、女1例，年龄57±6岁)；胫骨平台骨折、半月板损伤行关节镜清创术者4例(男3例、女1例，年龄35.5±2.1岁)。 术前抽取每一患者关节滑液2～5ml，4 000r/min离心30分钟，置1ml离心管分装，-50℃冷藏、待测。 2.滑液中TNF样物质生物活性检测：用L929细胞毒活性测定法〔3,4〕。重组人TNF-α标准品由上海生物化学研究所华新公司提供。测定结果以L929细胞杀伤率(KR)表示。 KR（％）＝〔阴性对照(OD)－标本(OD)〕÷〔阴性对照(OD)－阳性对照(OD)〕×100％ 阴性对照为L929细胞＋更生霉素D；阳性对照为L929细胞＋重组人TNF-α＋更生霉素D。 3.滑液中TNF-α特异性检测：将TNF-α单克隆抗体（军事医学科学院基础医学研究所）1∶10稀释后加入重组人TNF-α和待测滑液标本中，37℃作用1小时、4℃作用30分钟，2 000r／min离心10分钟，取上清，检测其L929细胞毒活性。 4.滑膜标本及处理：患者滑膜标本取材同滑液标本。无明显关节疾病的尸检滑膜标本2例，作对照。分不同部位，取滑膜组织2块(约1cm×1cm)，近内膜处取滑膜约1.5mm2，PBS漂洗，常规电镜制片，铅、铀双重染色，日立H-500型透射电镜观察；余下标本，常规电镜制片，HE和AS染色，光镜观察。结 果 一、滑液中TNF样物质的生物活性 在所有被检测的滑液标本中，均呈现L929细胞毒作用，但不同来源的标本，对L929细胞杀伤率不同（KR值为19.66％～230.84％）。以KR=90％为界，KR＞90％者：OA组7例（7/9）；股骨头无菌性坏死患者和股骨颈囊内骨折患者作为老年性慢性关节内损伤组（CAT）2例（2/11）；胫骨平台骨折和半月板损伤作为急性关节内损伤组（AAT）2例（2/4）（图1）。图1 三组患者关节滑液中TNF-α的KR值及发生频数示意图 采用四格表确切概率法（x2）进行组间统计学处理，OA组患者滑液所含TNF-α水平较高，对L929细胞毒作用的发生率明显高于CAT组及AAT组（P≤0.0215）。 二、滑液中TNF-α的特异性 滑液标本中加入1∶10稀释的TNF-α单克隆抗体后，其L929细胞毒活性物质可被完全或大部分中和，从而使KR值接近于阴性对照。 三、滑膜的形态学观察 1.滑膜组织学结构： (1)正常滑膜：滑膜层细胞性内膜由2～3层滑膜细胞组成，较疏松地平行排列，包埋在基质中；基质内的纤维成份由内向外逐渐增多，至内膜下层胶原纤维呈波浪状；细胞性内膜无明显基膜，因此与内膜下层分界不明显。滑膜下层结构疏松，含有较多的脂肪组织及纤维网状组织。 (2)急性胫骨平台骨折及半月板损伤滑膜：与正常滑膜比较，有较多血细胞为主要表现，血细胞多分布于内膜下层，但无多核性巨噬细胞。 (3)股骨颈囊内骨折滑膜和股骨头无菌性坏死滑膜：两者组织学特征呈现相似的变化，细胞性内膜细胞狭长，1～3层平行排列，其间基质成份稀少；内膜下层较薄，极少有细胞成分。 (4)OA滑膜：细胞性内膜细胞较大、变圆，平行排列；有的部位细胞层缺失，使内膜下层纤维呈绒毛状突出于滑膜内表面，而有的部位细胞层增多，可达6层，细胞间质成分较多(图2)；内膜下层增厚，以波浪状胶原纤维成分为主，有的部位呈束状(图3)；滑膜层（细胞性内膜至滑膜下层脂肪组织）可达0.50～0.55mm，纤维间缺乏细胞成分。 2.滑膜超微结构： (1)正常滑膜：正常滑膜细胞性内膜可分为A、B型细胞。A型细胞较多、形似巨噬细胞，细胞器以吞噬相为主要特征，包括发达的高尔基体，分散于胞质内的多种形式的小泡或囊泡和溶酶体；胞膜有较多细长的突起伸向间质；B型细胞形似成纤维细胞，细胞器以蛋白分泌相为主要特征，包括丰富的成层排列的粗面内质网，较多的高尔基体和线粒体。细胞间质为颗粒状或细丝状不定形纤丝，逐渐向内膜下层过渡为有周期性横纹的胶原纤丝。 (2)急性胫骨平台骨折和半月板损伤性滑膜：滑膜细胞以B型细胞为主，呈三角形或柱状；B型细胞蛋白分泌相旺盛，粗面内质网呈池状扩张，胞质内可见有脂滴及小泡；细胞膜较少突起；细胞周围有纵横排列的具周期性横纹的胶原纤丝；内膜下层胶原纤丝束状排列(图4)。 (3)股骨颈囊内骨折滑膜：滑膜细胞以A型滑膜细胞为主，细胞膜有不规则突起，细胞质内含有大量的溶酶体、丰富的高尔基体、囊泡及脂滴(图5)；B型细胞狭长，胞质内细胞器较少；细胞周围胶原纤丝稀疏(图6)。 (4)股骨头无菌性坏死滑膜：A型滑膜细胞体积较大，形态不规则，胞膜具有绒毛状或指状突起，胞质内含大量的溶酶体、吞噬物及囊泡(图7)；B型细胞形态不规则，胞膜突起较长，胞突内及细胞周围可见胶原纤丝。 (5)OA滑膜：滑膜细胞体积较大，A、B型细胞都呈现功能活跃相。A型细胞呈三角形，胞膜有微绒毛样突起，胞核染色质常凝集成团块状，细胞质内含大量的溶酶体及小泡(图8)；B型细胞呈极度活跃的蛋白分泌相，并含有较多的囊泡(图9)。细胞间常呈斑点和缝隙样连接。在滑膜细胞层内，尚可见三角形具较长突起的细胞，细胞质内较少细胞器，但含有丰富的中丝分布于核的周围。A、B型细胞周围具周期性横纹的胶原纤丝丰富。讨 论 1.在已知的细胞因子中，IL-1、TNF-α和IL-6是参与OA发病进程的重要介质。实验性OA的早期，TNF-α是滑液中最先出现的细胞因子，并与关节软骨的降解相关〔5〕。TNF-α既协同IL-1的作用，又可激活IL-6基因、诱导IL-6的生成，在OA的发生、发展中有可能起着决定性作用〔6〕。一般认为TNF-α可来源于由单核细胞等浸润的炎性滑膜组织及变性软骨细胞。然而，本研究发现，除变性的软骨细胞等因素外，晚期OA与其滑液中TNF-α含量增加相关的滑膜组织并非表现为明显的炎性病变，而以A、B型滑膜细胞功能异常活跃为主。因此，可以认为滑液中TNF-α的异常升高主要归因于滑膜A、B型细胞的功能异常。 2.OA通常被分为原发性和继发性，但两者并无明显的病理学差异。继发性OA可由多种原因所诱发，关节创伤几乎不可避免地引起OA〔7〕。既往的动物实验表明，单一的急性滑膜炎症并不能诱发OA，只有对滑膜炎性病变的关节施加可致关节不稳定性因素时，才能导致OA的发生〔8〕。本研究发现，因胫骨平台骨折和半月板损伤所造成的关节内创伤，B型滑膜细胞数量增加、功能旺盛，从而使内膜下层胶原纤维含量丰富。这种现象在晚期OA滑膜组织同样发生，并且滑膜组织的纤维化程度及厚度增加更为明显。提示其间可能存在的某些因果关系。关节创伤后，一方面机体对损伤后的保护性限动，加速关节软骨的退变〔7〕；另一方面滑膜组织细胞因对创伤的反应性而出现异常增殖和功能改变，使关节滑液出现异常蛋白分子（包括细胞因子等），进而刺激滑膜细胞的增殖〔9〕，使关节力学特征因滑膜纤维化(甚至瘢痕化)而改变，关节软骨细胞生活微环境进一步向非生理方向变化，形成恶性循环，结果导致关节软骨细胞变性，呈现OA的病理特征。这其间更复杂的机理有待进一步阐明。 3.关节软骨的退变及变性软骨细胞的产物，可促进滑膜细胞的增殖和功能。股骨颈囊内骨折和股骨头无菌性坏死，作为关节内亚急性及慢性创伤的病因，本研究未发现滑膜组织明显的纤维性增生现象，坏死组织主要刺激A型滑膜细胞，使其数量增加、吞噬功能活跃。而在关节软骨并不坏死的情况下，关节内损伤早期及晚期OA，B型滑膜细胞功能异常旺盛、蛋白分泌相活跃；在OA，因脱落关节软骨碎片等异物刺激，A型滑膜细胞同时亦呈现功能活跃相。提示滑膜细胞，尤其是B型滑膜细胞的生物学活性提高，可能由变性软骨细胞的分泌物所介导。这种介导作用还表现为，OA滑膜组织内，有些A型滑膜细胞质内含有丰富的中丝，这可能与细胞的迁移、转化有关〔10〕。图2 骨关节炎滑膜组织，细胞性内膜厚薄不均，内膜下层细胞成分极少(HE×62.5) 图3 骨关节炎滑膜组织，内膜下层胶原纤维呈波浪状或束状排列(AS×130) 图4 急性胫骨平台骨折滑膜B型细胞，呈三角形、蛋白分泌相旺盛(TEM×5000) 图5 股骨颈囊内骨折，A型滑膜细胞含大量吞噬囊泡及高尔基体，细胞膜呈指状突起(TEM×5000) 图6 股骨颈囊内骨折，B型滑膜细胞狭长，细胞质内细胞器较少(TEM×5000) 图7 股骨头无菌性坏死，A型滑膜细胞含大量的吞噬异物(TEM×5000) 图8 骨关节炎，A型滑膜细胞膜微绒毛状突起，细胞质内含大量溶酶体及小泡(TEM×4000) 图9 骨关节炎，B型滑膜细胞，呈蛋白分泌旺盛相，细胞周围有大量的胶原纤丝(TEM×4000)参 考 文 献 1 Pelletier JP, Di Battista JA, Roughleg P, et al.
