点击上方“蓝色微信名”可订阅哦！ 很多家长对奥司他韦这个药都不陌生，尤其是在流感肆虐的季节，医生经常会给感冒的宝宝处方这个药。有些家长听说这个抗流感的神药，甚至会想办法在家储备一些，一旦感冒了就给宝宝吃上或用它来预防感冒；但也有的家长看到说明书中列举的种种不良反应，心中担心而不敢给宝宝吃；有的家长在疑惑，为什么给孩子吃上奥司他韦了，他/她还继续发烧呢？还能给宝宝吃退烧药或别的药吗？能和抗生素一起吃吗？1岁以下的宝宝能吃奥司他韦吗？……如果您也有这样的疑惑，可以看看这篇药师整理的关于奥司他韦常见问题的一些解答。 01 奥司他韦是什么药？ 奥司他韦是一种抗 流感病毒 药物，能阻止流感病毒在人体内复制和致病性，减轻流感症状，减少甲型或乙型流感病毒的传播。因此奥司他韦可用于甲型和乙型流感的治疗和预防。（这里要画一下重点的是，奥司他韦主要用于治疗 甲型和乙型 流感，而不是对所有的病毒都有效，要注意抗流感病毒和抗病毒的区别。 02 多大的孩子可以吃奥司他韦？怎么服用 中国药品说明书标示的是：奥司他韦用于成人和 1岁及1岁以上 儿童甲型和乙型流感的 治疗 （患者应在首次出现症状48小时以内使用）；用于成人和 13岁及13岁以上 青少年 甲型和乙型流感的 预防 。 但美国食品药品监督管理局（FDA）批准使用奥司他韦治疗2周龄（14天）以上，有流感症状不超过48小时的儿童。（需要注意的是用于预防甲型和乙型流感病毒感染，还是要求用药者大于等于1岁。）根据美国疾病预防与控制中心(CDC)的数据，推荐<1岁的患儿奥司他韦口服治疗剂量为3mg/kg，2次/d。早产儿由于肾功能不成熟，对口服奥司他韦的清除较慢，对足月儿推荐的剂量在早产儿中可能导致药物浓度过高，需要根据矫正胎龄来计算用药剂量：矫正胎龄<38周、38~40周和>40周的婴儿奥司他韦的治疗剂量分别为“1.0mg/kg，2次/d”、“1.5mg/kg，2次/d”和“3.0mg/kg，2次/d”。 2016年，中华医学会呼吸病学分会和中华医学会儿科学分会制定的《流行性感冒抗病毒药物治疗与预防应用中国专家共识》中也推荐了1岁以下儿童的用法用量，因此奥司他韦还是可以用于1岁以下的婴幼儿的。下表是专家共识中推荐的各年龄段预防和治疗流感服用奥司他韦的剂量： 03 得了流感就一定要吃奥司他韦吗？ 我们知道，普通感冒是一种自限性的疾病，也就是说大部分的人，如果症状轻的话，不用吃药也能好。流感也可以认为是升级版的普通感冒，不吃奥司他韦大多也会自愈。服用奥司他韦可以缩短病程，可能会减少并发症（比如中耳炎）的发生和减轻疾病的严重程度。所以，对于并发症发生风险低的非高危人群，可以根据自身的情况来判断要不要服用奥司他韦。但对于重症流感的高危人群，建议还是尽早（48小时以内用药效果最佳）服用奥司他韦，以免进展为重症流感或发生并发症。 根据中华人民共和国卫生和计划生育委员会发布的《流行性感冒诊疗方案（2018年版）》，年龄＜5岁的儿童（年龄＜2岁更容易发生严重并发症）和有基础疾病、免疫功能低下的患者为重症流感的高危人群，最好还是尽早服用奥司他韦。 04 什么时候服用奥司他韦合适？ 细心的家长会发现，说明书中有提到患者应在首次出现症状48小时以内使用奥司他韦，为什么呢？如果超过48小时，还能再用吗？ 因为奥司他韦只能抑制病毒的复制，而不能杀灭病毒，也就是说已经产生出来的病毒还是要靠人体的免疫系统将其清除，但如果没有新的病毒产生的话，随着之前的病毒慢慢被消灭，感冒也就好了。早期被病毒感染的细胞数量还很少，病毒感染就被限制在较小的范围内。所以如果怀疑和确诊流感，在症状出现后，越早服用奥司他韦效果越好，可以缩短症状持续时间，同时减少一些并发症的发生。理论上认为48小时病毒已经基本完成了增殖，如果已经超过48小时，服药带来的益处大大减小。 因此，对于症状轻微、或明显已好转的孩子，48小时后再服用奥司他韦获益不大，不服药也可以自愈。 但对于重症的孩子，或者症状一直无改善或恶化的孩子，即使是症状超过了48小时，仍然可以考虑使用。也可以从奥司他韦中获益，也是可以吃的。 出现以下情况之一者为重症病例 1.持续高热>3天，伴有剧烈咳嗽，咳脓痰、血痰，或胸痛； 2.呼吸频率快，呼吸困难，口唇紫绀； 3.神志改变：反应迟钝、嗜睡、躁动、惊厥等； 4.严重呕吐、腹泻，出现脱水表现； 5.合并肺炎； 6.原有基础疾病明显加重。 05 奥司他韦可以和退烧药或抗生素一起吃吗？ 如果服药期间孩子体温＞38.5℃或/和有头痛、全身肌肉酸痛等不适表现，可给予布洛芬或对乙酰氨基酚解热镇痛，注意避免使用水杨酸类药物。水杨酸类药物（比如阿司匹林）和瑞氏综合征的发生有关，尤其同时有病毒感染性疾病（比如流感）。 抗生素对于病毒是无效的，如果只是单纯流感，不需要服用抗生素。但如果医生诊断有合并细菌感染，则需要加用抗生素治疗。 06 如果孩子吃了一天奥司他韦很快就退烧了，还需要再继续吃够5天吗？ 奥司他韦对于流感的治疗，疗程为5d。如果提前停药，病毒可能会继续复制、繁殖，造成病情反复；也可能会推进流感病毒对奥司他韦的耐药，而且此时仍具有传染性，因此建议仍需吃够疗程。 07 奥司他韦怎么服用？有什么副作用？ 奥司他韦可伴或不伴食物服用，与食物同服可减少胃肠道不良反应，增加用药耐受性。 奥司他韦最常见的副作用是胃肠道不适，主要是恶心、呕吐等，大多可自行缓解；少数病人也可能出现皮疹、神经精神症状等，若出现或疑似过敏样反应，应停药并给予适当治疗。若出现神经精神症状，应由专业的医务人员权衡利弊后决定是否继续用药。 08 奥司他韦能替代流感疫苗吗？ 不能，本药仅在用药期间发挥流感预防作用，鉴于该药的不良反应，不能长期用药，故不可取代流感疫苗。只有以下儿童在流感流行期间或与流感患者接触后可考虑抗病毒药物预防流感： 1）接种疫苗后尚未建立稳定免疫力(接种后2周内)的高危儿童； 2）因禁忌证未接种疫苗或接种疫苗后难以获得有效免疫力(如各种原因所致的严重免疫缺陷、所接种疫苗株与此次流行毒株抗原差距性较大)的高合并症风险儿童； 3）与流感并发症高危人群密切接触的≥1岁儿童。（文/钟旭丽） 首都儿科研究所 服务号 首都儿科研究所 科普号 首都儿科研究所 附属儿童医院
