一、概述 　　21-三体综合征(21-trisomy syndrome)又称先天愚型或Down综合征，是常染色体疾病中最常见的一种遗传性疾病。患儿的细胞染色体中第21对多了1个，其发病率在出生婴儿中为1/700，发病男＞女，男女之比为3:1。 　　21号染色体三体是生殖细胞在减数分裂过程中，由于某些因素的影响发生不分离所致，其发生率在出生婴儿中为1.45‰，或约为1/700，实际发病率还高，因约一半以上病例在妊娠早期即自行流产，男女之比为3:2。病因与母亲妊娠时的年龄、遗传因素、妊娠时使用化学药物堕胎、放射线照射、自身免疫性疾病（如慢性甲状腺炎）及病毒感染（如传染性肝炎）等有关。21三体综合征病儿随母亲年龄增大而发病率增高，母亲年龄在35岁者，子女的发病率为1/300，40～45岁为1/100，45岁以上为1/50。发病高峰与母亲年龄有明显关系，有的作者设想是因卵子衰老所致。病例大多为散在发生。21三体综合征病儿染色体的过氧化物歧化酶-I（简称SOD-1），活性增加，正常人SOD-1活性为1.0，21单体为0.5，21三体为1.5。SOD-1活性为1.5时，有典型临床表现，嵌合体病儿临床表现可不典型。 　　本病没有典型的病理改变，主要变化是大脑和小脑的发育异常，大脑可能不对称，皮质变薄，视丘下部发育不全，小脑缩小，部分脑沟发育不正常。伴发的畸形包括心房下部间隔缺损、心室间隔缺损或房室联合缺损、十二指肠闭锁、膈疝、脐疝等。学龄儿可见具有特殊斑点的板层性白内障。 　　二、临床表现 　　病儿智能落后，肌张力低下，皮肤呈大理石样色泽。 　　1.头面部：生后不久即可见特殊面容，可有第三囟门，短小头型，面圆而扁平，与鼻骨及上颌骨发育不良有关。眼球较突出，睑裂明显的斜向外上，两眼距离较远，眼裂内角内眦赘皮（此特点只对白种人有用，因我国正常人也多有，故不能靠它做诊断）。眼球震颤，少数有白内障。眼睫毛稀疏而短，常见睑缘炎及斜视。虹膜有白斑，特别是当虹膜呈蓝色的时候出现（Brushfield点，为小圆的白点，略不规则，在虹膜中，外1/3交界处形成一个环）。鼻根由于鼻梁骨发育不良而显低平，鼻子短，鼻孔上翘。嘴小，口半开，腭弓高，唇厚，舌厚常伸出口外，可见舌炎及表皮脱落，年龄较大病儿可见裂纹。有的病儿有唇裂、腭裂，流涎多，傻笑，牙齿萌出延迟。总的印象是很愚钝。耳小而圆，耳轮上缘过度折叠，耳长度减小，耳垂小、粘连。耳轮根部异常发育下移至耳甲部，外耳道小。 　　2.胸腹部：颈背部短而宽，50%可伴有先天性心脏病，依出现多少顺序为房室联合通道、室间隔缺损、房间隔缺损、动脉导管未闭、法洛氏四联症等。由于肌张力低，腹膨隆，常伴有腹直肌分离和脐疝。消化道畸形主要为十二指肠狭窄，也可见食管气管瘘、膈疝、幽门狭窄、环状胰腺、肛门闭锁、巨结肠、直肠脱垂等。 　　3.四肢：病儿有重度肌张力低下，四肢关节柔软，可任意攀翻，手宽、厚，指短，小指末端常向内弯，它的中间的指骨较正常短而宽。脚宽、厚，（足母）趾与其余四趾分离较远。 　　4.骨骼：有明显的骨异常，常见骨盆小，X线片髂翼伸展，髋臼扁平，坐骨削尖，髋臼指数（髋臼角与髋骨角之和）＜68°。小指中节及末节指骨发育不良，X线可见脱钙。第一掌骨的远端和第二掌骨的近端常有不整齐的凹痕。胸骨柄可见2个骨化点，第12对肋骨缺如。额窦消失，额缝持续存在，骨龄稍延迟。 　　5.智能障碍：智能不足的程度范围很大，一般5岁时智商为50，渐渐减退，至15岁时为38。缺乏抽象思维能力。嵌合体型病儿智能水平多高于21三体型的患儿，智商平均可达67。一些嵌合体病儿语言发育较好，视觉理解障碍较少，有的病儿智能接近正常。 　　6.皮肤纹理：掌纹只有一条，呈通贯手或悉尼线，轴三射远位呈t″，atd角增大（＞63°，正常＜45°），小指只有一条褶纹，指纹斗状纹频率减少，箕状纹增加。无名指及小指挠向箕纹增多，食指尺侧箕纹增多，（足母）趾球部胫侧弓形纹。 　　7.其他：患儿身高、体重低于正常，出生时平均身长和体重均较正常婴儿低。易患呼吸道感染、消化障碍及睑缘炎。有时可见心血管兴奋性增高、心力衰竭、体温调节障碍（常是体温过低）等表现。气脑造影有时能发现蛛网膜下腔扩大及两侧对称性的脑室扩张。10%可有癫痫，患白血病率较正常儿约高20倍左右。 三、医技检查 　　1.染色体检查 　　⑴21-三体型：染色体组的核型为47XX（或XY）＋21，占90%～95%。患儿染色体总数为47个，是由于多了一个小的端着丝点染色体（属于第21对），这种现象称为三体性，因为有一对染色体变成了三个。母亲年龄大于35岁时发生机率明显增高（约为群体发病率的125倍），双亲的染色体并无异常，也无家族史，所以认为是母亲的配子（卵子）发生染色体不分离的结果。约1/5病例由父亲染色体不分离所致。在一卵性双胎时，如果一个是21-三体综合征，则在另一个孩子100%也是21-三体综合征。而在二卵性双胎时，二儿同是21-三体综合征的机会只有30%。 　　⑵易位型：少数病例的染色体总数为46个，21号染色体仍具有三体性的特征，同时还伴有其他染色体的畸变。有几种不同情况：①D/G易位：缺少了一个14号染色体，增添了一个新的染色体，后者是由一个21号与一个14号染色体相互易位而连接成一个新的14/21染色体。此染色体保留了14、21号染色体绝大部分的遗传物质，所以实际上此患儿的细胞内仍为3个21号染色体（三体性），染色体组核型为46XX（或XY）＋21，t（Dq21q）。此类患儿占2%，其母亲外表正常，但其细胞的染色体数为45个，其中有一个是14/21染色体。患儿的姨母及姨表兄妹可能同患此病。②G/G易位：21与22号染色体易位，形成一个21/22染色体，可能是由患者的父亲所遗传的；两个21号染色体所连接成的一个等臂染色体，还有一个正常的21号染色体，如母为携带者，其子代100%为患儿。染色体组核型为46XX（或XY）＋21，此种病儿占2%。 　　⑶嵌合体型：占1%～2%，染色体组核型为47XX（或XY）＋21/46XX（或XY）。较少见，病儿细胞内染色体各有不同，有的为46，有的为47或45个。此型主要是受精卵在卵裂期发生差错所致。有些21-三体征患者的父母表现型正常，而具有一个21-三体的嵌合体，母亲为嵌合体者占大多数，父亲较少。如果一个家庭中出现2个以上21-三体征，可能与其父母为嵌合体型有关。 　　2.代谢障碍：由于色氨酸代谢障碍，黄嘌呤尿酸从泌尿道的排泄率明显减少，血小板单胺氧化酶活性降低，出现高尿酸血症。 　　3.其他 　　⑴酶障碍：白细胞碱性磷酸酶活性增加，其基因位于21号染色体上，G-6-PD增加。过氧化物歧化酶-1（SOD-1）活性增加。 　　⑵免疫球蛋白异常，IgG减少，澳抗阳性的百分率增高。 　　⑶可有血红蛋白F及A2增高。 　　21-三体综合征病儿对阿托品、匹罗卡品、麻黄素敏感，可能与胆碱性神经功能障碍有关。 　　四、诊断依据 　　1.母亲多为高龄产妇或近亲结婚。 　　2.典型的临床表现 　　⑴特殊面容：如眼距宽，两眼外眦上斜，鼻梁平，耳廓小，硬腭窄，口半张，舌常伸出口外，流涎。 　　⑵智力障碍，智商多在25%～50%。 　　⑶肌张力低下，关节过伸，手指短而粗，小指较短且向内侧弯，皮纹异常。 　　⑷出牙迟，顺序异常，牙呈楔形，牙釉质发育不全。骨龄发育迟缓。 　　3.半数患儿有先天性心脏病或偶有脐疝、小阴茎、隐睾等畸形。 　　4.X线骨骼检查：可见多中心钙化，新生儿可见胸骨多中心钙化，小指中、末节发育不良，可有第十二肋缺如。 　　根据特殊面容、异常体征和智能落后容易诊断。有的21-三体综合征病儿表现在出生时即很显著，有的体征到1岁时才明显。有时怀疑为21-三体综合征，但依靠临床不能确定，此时染色体检查特别有帮助。 　　新生儿病例的诊断：有的病例出生时症状不明显，但患儿平静无气力地躺着，不似正常儿那样活动，面部无表情，面色苍白，对周围环境无兴趣，似粗笨样（可能由于皮肤水肿），皮肤呈大理石样。大腿松弛向两侧外展，哭声微弱短促，膝反射弱，Moro反射消失，肌张力低，自主能力减退，对刺激的反应呆钝，说明中枢神经系统的活动受抑制。 　　五、容易误诊的疾病 　　1.假先天愚型：Hall氏研究43例被怀疑为21-三体综合征的新生儿，其中38例有相应的染色体异常，5例为正常染色体，其中1例在出生时具有2项21-三体综合征的主要临床症状，另1例具有4项，但到1岁时这些症状都消失。1例出现为母孕期应用酞胺哌啶酮或名反应停（Thalidomide）所引起；其余2例在新生儿期各有4项主要症状，当1岁时出现智能迟钝，且这些表现持续存在。Hal1氏将其称为假先天愚型。 　　2.甲状腺功能低下。 　　六、治疗原则 　　无特效药物，以进行长期耐心的教育和训练为主，采用综合措施，包括医疗和社会服务。先天性心脏病可行外科或内科治疗。胃肠道或其他畸形外科治疗。甲状腺功能低下病儿用甲状腺素治疗，听力障碍可用助听器。给予大量维生素、γ-氨酪酸、谷氨酸口服，补充适量的微量元素，似有稳定肌力和促进智力的作用。应对弱智儿进行预备教育以使其能过度到普通学校上学，训练弱智儿掌握一定的工作技能；此外家长和学校应帮助孩子克服行为问题，社会也应对残疾儿的父母给予道义上的支持。 　　七、预防及预后 　　过去报告成活率低，常在1岁内夭折，近年由于抗生素和心外科手术的发展，死亡率下降，75%可活到5岁，50%活到中年。寿命长短取决于有无并发症，先天性心脏病是早期死亡的主要原因，同时患严重呼吸道感染时常导致心力衰竭而死亡，应加以警惕。智力随年龄增长而下降，平均IQ2岁为56，4岁为46，11岁为37。女性IQ一般高于男性，在家生活的孩子IQ一般高于教养院生活的孩子。出生时体重及肌张力低是一有用的预测指标，嵌合体患儿少有较重的影响，有些孩子可以学习、读和写；有的孩子则不会说话，13%有严重的行为问题。视力障碍包括斜视、近视、眼球震颤、失明和白内障。常见传导性耳聋。14%有隐睾症、性欲低下、精子量减少，男性可能不育，女性病人和正常人结婚后也有怀孕者。 　　根据病因方面的初步了解，可针对以下情况进行预防：①妇女应避免在40岁以后生育；②25～30岁以下的母亲如生有21三体综合征病儿时，应检查母亲的染色体，30岁以下母亲再生出此综合征患儿的危险较大，因8%～9%的易位型三体征是30岁以下母亲所生，如母亲染色体检查有D/G易位畸变，应以节育为好。如这样母亲已怀第2胎，可做产前羊水穿刺检查，进行羊水细胞培养，检查胎儿染色体核型，如患儿染色体异常，可终止妊娠；③对母亲为21-三体征嵌合体型者，妊娠时可做羊水穿刺检查；④妊娠期间，尤其早期应避免用化学药物打胎或服用磺胺药类以及X线照射，预防传染性肝炎的发生。
