维生素E是6-羟基苯并二氢吡喃环的异戊二烯衍生物，包括生育酚和生育三烯酚（tocotrienol）两类共八种化合物，即数学公式：α、β、γ、δ生育酚和数学公式：α、β、γ、δ生育三烯酚。数学公式:-生育酚是自然界中分布最广泛、含量最丰富、活性最高的维生素E[1]。 普遍认为维生素E的功能是抗氧化，为生殖系统必需，对免疫和神经系统有作用。 VE还可以提高体液免疫的抗体反应，但并非以直接的方式来提高[8]。 HPV：根据研究发现维A、维D可以有效减缓CIN1的进展，叶酸B12可以减缓CIN2的进展，维E、叶酸B12、番茄红素可以减缓CIN3的进展，类胡萝卜素可以预防HPV感染和减缓CIN的进展[10][11]。 天然VE是一种强抗氧化剂，可预防或减轻细胞膜脂质过氧化损伤和预防油脂的氧化。 细胞膜脂质过氧化损伤：细胞膜上含有丰富的PUFAs，易受到自由基的攻击而发生脂质过氧化反应。而脂质过氧化反应是一种自由基介导的链式反应过程，如果此过程中有生育酚介入，则可中断链式反应。油脂的自动氧化：油脂的氧化主要有自动氧化，光氧化反应，酶催化反应，金属催化氧化反应四种类型。其中自动氧化是食用油脂和油基食品劣化的主要原因，自动氧化过程十分复杂，它是一个自由基链式反应（与上面不同）。VE可以提供氢原子使ROO·（或者RO·）形成稳定的化合物[12][13]。 黑色素形成的生理基本过程可概括为：黑色素细胞产生黑色素，黑色素颗粒通过黑色素细胞枝状突起向角质细胞转移，转移至角质细胞的黑色素颗粒随表皮细胞上行至角质层，从而影响皮肤的颜色并形成色斑，最终随角质层脱落而排泄。值得注意的是，在基底层产生的黑色并不能引起视觉上的“变黑”，只有当黑色素被转移到角质层，才能引起肉眼可见的“变黑”。酪氨酸在黑色素细胞内通过酪氨酸酶及多种氧化酶的催化生成黑色素，产生的黑色素一部分上移至角质层并随之代谢脱落，而另外一部分黑色素则下移被皮肤内毛细血管吸收，随血液循环从肾脏排出。 维E与维C相似可以抑制UV导致的黑色素生成并抑制黑色素细胞的增殖，并且还可以抑制酪氨酸酶及酪氨酸酶相关蛋白质-2 mRNA的表达[14][15]。 维生素C提高机体免疫力主要通过两方面的作用： 白细胞的吞噬功能依赖于血浆维生素C水平。体内维生素 C 的含量高时，白细胞更加活跃，清除病原菌的能力更强。 通过抗氧化作用促进抗体形成，在抗体分子中含有相当数量的二硫键，这些二硫键由2个半胱氨酸构成，较高浓度的维生素C能通过使二硫键还原为巯基 。促进食物中的胱氨酸还原为半胱氨酸，从而形成抗体。 维生素C能诱导体内产生干扰素，干扰病毒 mRNA 转录和 DNA 复制，增强对病毒的抵抗力。维生素 C 可增强中性粒细胞的趋化性和变形能力，提高杀菌效果。 维生素C可通过增加产生干扰素IFN-α/β， 在病毒感染，特别是对流感病毒的感染有良好的抗病毒免疫反应，可通过促进淋巴母细胞的生成，提高机体对外来和恶变细胞的识别和杀灭，参与免疫球蛋白合成，有效地提高机体免疫力。 α-生育酚是自然界中分布最广泛、含量最丰富、活性最高的维生素E。天然维生素E的功效成分标示为d-α-生育酚，而人工合成的产品则标示为dl-α-生育酚。从吸收率和生理活性上来讲，天然维生素E比人工合成的维生素E要高，有关实验研究证明，天然维生素E比人工合成的吸收率高出3.5倍。但从稳定性来讲，人工合成的维生素E稳定性相对更好。 然而近来一些研究表明γ-生育酚也同样有重要功能。最强的氧化剂为过氧化亚硝酸盐、NO2、NO。如下图所示，gamma的NO解毒能力（抗自由基）要比alpha强，gamma的解毒产物基本无害，alpha的解毒产物是可能致突变的亚硝基产物。Gamma可以中和已经存在的自由基，而alpha只能预防新的自由基出现。 混合生育酚在抑制血小板聚集方面比α-生育酚更有效。接受富含γ-生育酚的维生素E（每天100mg gamma，40mg delta和20mg alpha）的人的二磷酸腺苷诱导的血小板聚集比单独接受纯α-生育酚的人显着降低。研究表明每天100mg gamma的摄入可以有效降低心血管疾病的风险和胆固醇的水平。此外gamma还表现出对一型糖尿病有一定的保护作用，其余的功能与alpha大致一致[14][16]。 Gamma还对癌症有一定预防作用，通过摧毁RNS，刺激p53，诱导癌细胞凋亡，调节可影响癌变的相关基因的表达可使gamma一定程度上预防结肠癌和乳腺癌[15]。Gamma的效力强于别的生育酚主要得益于其化学结构，它的结构能让它高效的结合并阻止PKB（过度表达会使细胞增殖并癌变）[19]。 在脂溶性维生素中，维生素E的毒性相对较低，动物实验未见维生素E有致畸、致癌、致突变作用，人和动物均可耐受需求量二倍以上的剂量。然而，极高剂量维生素E可与其他指溶性维生素维生素（维生素A、D和K）产生拮抗作用。动物实验发现大剂量维生素E可抑制生长，干扰甲状腺功能及血液凝固、使肝脏中脂类增加。大多数成人都可以耐受每日口服100-800mg的维生素E而没有明显的毒性症状和生化指标的改变。使用抗凝药物或有维生素K缺乏的人，但没有密切医疗监控情况下不宜使用维生素E补充剂，因为有增加出血致命的危险。 根据各国推荐摄入量以及摄入上限量，本品268+67mg/d安全合理并且想有健康收益剂量需大于200mg[16]。 给动物喂饲不含维生素E的合成饲料可引起维生素E缺乏症，主要表现为生殖障碍、神经肌肉障碍、血浆中维生素E浓度降低、红细胞膜受损、红细胞寿命缩短以及溶血性贫血。维生素E在自然界中分布甚广，一般情况下人体不会因为摄入不足而导致缺乏。但是在α-TTP或脂蛋白合成基因异常的人群中，明显的维生素E缺乏还时有发生。其他导致维生素E缺乏的疾病还有囊性纤维性变性（婴儿的一种遗传性症候群及吸收不良）、慢性胆汁郁积性肝病、短肠综合征以及其他形式的慢性腹泻，脂肪吸收不良、其血浆维生素E明显减少。维生素E缺乏主要影响脊索的后柱、第三和第四脑神经核、周围神经的大髓鞘轴突管、脑干的细长核，最后是肌肉和视网膜。因此维生素E缺乏的典型神经体征包括：深层键反射丧失、震颤和位感受损、平衡与协调改变、眼移动障碍（眼肌麻痹）、肌肉软弱和视野障碍。 成年人已成熟的神经系统对维生素E缺乏比较耐受，一般5-10年后才会出现神经方面的异常。但是儿童发育中的神经系统对维生素E缺乏很敏感，当维生素E缺乏时，如不及时使用维生素E补充治疗，可很快出现神经系统的异常症状，并影响认知能力和运动发育。早产儿出生时血浆和组织中维生素E水平很低，而且消化器官不成熟，多有维生素E的吸收障碍，往往容易出现溶血性贫血，肌肉注射维生素E可以改善症状。 副作用：作为补充生理剂量的维生素E长期过量服用可引起恶心、呕吐、眩晕、头痛、视力模糊、皮肤皲裂、腹泻、乳腺肿大、乏力等，由于它具有抗凝作用，长期大剂量（1000mg）摄入，还可增加出血性卒中风险[2][17]。 禁忌症：不可与血液稀释剂如阿司匹林、华法林等一起使用，也不可与他莫昔芬（一种用于治疗乳腺癌的抗雌激素）、环孢霉素（一种用于治疗器官移植的免疫抑制剂）一起使用[3]。 References
