透析(dialysis)是通过小分子经过半透膜扩散到水(或缓冲液)的原理，将小分子与生物大分子分开的一种分离纯化技术。透析疗法是使体液内的成分(溶质或水分)通过半透膜排出体外的治疗方法，一般可分为血液透析和腹膜透析两种。用于医学上的透析大致分为三大类:血液透析、腹膜透析、结肠透析。折叠血液透析血液透析(Hemodialysis)，简称血透，通俗的说法也称之为人工肾、洗肾，是血液净化技术的一种。其利用半透膜原理，通过弥散、对流体内各种有害以及多余的代谢废物和过多的电解质移出体外，达到净化血液的目的，并达到纠正水电解质及酸碱平衡的目的。血液透析所使用的半透膜厚度为10~20微米，膜上的孔径平均为3纳米，所以只允许分子量为1.5万以下的小分子和部分中分子物质通过，而分子量大于3.5万的大分子物质不能通过。因此，蛋白质、致热原、病毒、细菌以及血细胞等都是不可透出的;尿的成分中大部分是水，要想用人工肾替代肾脏就必须从血液中排出大量的水分，人工肾只能利用渗透压和超滤压来达到清除过多的水分之目的。所使用的人工肾即血液透析装置都具备上述这些功能，从而对血液的质和量进行调节，使之近于生理状态。

　　血液透析是急慢性肾功能衰竭患者肾脏替代治疗方式之一。那么你对血液透析了解多少呢?以下是由我整理关于血液透析知识的内容，希望大家喜欢! 　　血液透析的原理 　　溶质转运 　　1、 弥散：是HD时清除溶质的主要机制。溶质依靠浓度梯度从高浓度一侧向低浓度一侧转运，此现象称为弥散。溶质的弥散转运能源来自溶质的分子或微粒自身的不规则运动(布朗运动)。 　　2、 对流：溶质伴随溶剂一起通过半透膜的移动，称为对流。溶质和溶剂一起移动，是摩擦力作用的结果。不受溶质分子量和其浓度梯度差的影响，跨膜的动力是膜两侧的静水压差，即所谓溶质牵引作用。 　　3、 吸附：是通过正负电荷的相互作用或范德华力和透析膜表面的亲水性基团选择性吸附某些蛋白质、毒物及药物(如u03b22-微球蛋白、补体、炎性介质、内毒素等)。所有透析膜表面均带负电荷，膜表面负电荷量决定了吸附带有异种电荷蛋白的量。在血透过程中，血液中某些异常升高的蛋白质、毒物和药物等选择性地吸附于透析膜表面，使这些致病物质被清除，从而达到治疗的目的。 　　水的转运 　　1、 超滤定义：液体在静水压力梯度或渗透压梯度作用下通过半透膜的运动称为超滤。透析时，超滤是指水分从血液侧向透析液侧移动;反之，如果水分从透析液侧向血液侧移动，则称为反超滤。 　　2、 影响超滤的因素 　　(1) 净水压力梯度：主要来自透析液侧的负压，也可来自血液侧的正压。 　　(2) 渗透压梯度：水分通过半透膜从低浓度侧向高浓度侧移动，称为渗透。其动力是渗透压梯度。当两种溶液被半透膜隔开，且溶液中溶质的颗粒数量不等时，水分向溶质颗粒多的一侧流动，在水分流动的同时也牵引可以透过半透膜的溶质移动。水分移动后，将使膜两侧的溶质浓度相等，渗透超滤也停止。血透时，透析液与血浆基本等渗，因而超滤并不依赖渗透压梯度，而主要由静水压力梯度决定。 　　(3) 跨膜压力：是指血液侧正压和透析液侧负压的绝对值之和。血液侧正压一般用静脉回路侧除泡器内的静脉压来表示。 　　(4) 超滤系数：是指在单位跨膜压下，水通过透析膜的流量，反映了透析器的水通过能力。不同超滤系数值透析器，在相同跨膜压下水的清除量不同。 　　血液透析设备 　　血液透析的设备包括血液透析机、水处理及透析器、共同组成血液透析系统。 　　1、 血液透析机：是血液净化治疗中应用最广泛的一种治疗仪器，是一个较为复杂的机电一体化设备，由透析液供给监控装置及体外循环监控装置组成。它包括血泵，是驱动血液体外循环的动力;透析液配置系统;联机配置合适电解质浓度的透析液;容量控制系统，保证进出透析器的液体量达到预定的平衡目标;及各种安全监测系统，包括压力监控、空气监控及漏血监控等。 　　2、 水处理系统：由于一次透析中患者血液要隔着透析膜接触大量透析液(120L),而城市自来水含各种微量元素特别是重金属元素，同时还含一些消毒剂、内毒素及细菌，与血液接触将导致这些物质进入体内。因此自来水需依次经过滤、除铁、软化、活性炭、反渗透处理，只有反渗水方可作为浓缩透析液的稀释用水。而对自来水进行一系列处理的装置即为水处理系统。 　　3、 透析器：也称“人工肾”，由一根根化学材料制成的空心纤维组成，而每根空心纤维上分布着无数小孔。透析时血液经空心纤维内而透析液经空心纤维外反向流过，血液/透析液中的一些小分子的溶质及水分即通过空心纤维上的小孔进行交换，交换的最终结果是血液中的尿毒症毒素及一些电解质、多余的水分进入透析液中被清除，透析液中一些碳酸氢根及电解质进入血液中。从而达到清除毒素、水分、维持酸碱平衡及内环境稳定的目的。整个空心纤维的总面积即交换面积决定了小分子物质的通过能力，而膜孔径的大小决定了中大分子的通过能力。 　　4、 透析液：透析液由含电解质及碱基的透析浓缩液与反渗水按比例稀释后得到，最终形成与血液电解质浓度接近的溶液，以维持正常电解质水平，同时通过较高的碱基浓度提供碱基给机体，以纠正患者存在的酸中毒。常用的透析液碱基主要为碳酸氢盐，还含少量醋酸。 　　血液透析的适应与禁忌 　　适应症 　　一、 急性肾损伤：凡急性肾损伤合并高分解代谢者(每日血尿素氮BUN上升u226510、7mmol/L，血清肌酐SCr上升u2265176、8umol/L，血钾上升1-2mmol/L，HCO3-下降u22652mmol/L)可透析治疗。非高分解代谢者，但符合下述第一项并有任何其他一项者，即可进行透析：①无尿48h以上;②BUNu226521、4mmol/L;③SCru2265442umol/L;④血钾u22656、5mmol/L;⑤HCO3-<15mmol/L，CO2结合力L;⑥有明显水肿、肺水肿、恶心、呕吐、嗜睡、躁动或意识障碍;⑦误输异型血或其他原因所致溶血、游离血红蛋白>12、4mmol/L。决定患者是否立即开始肾脏替代治疗，及选择何种方式，不能单凭某项指标，而应综合考虑。 　　二、 慢性肾功能衰竭：慢性肾功能衰竭血液透析的时机尚无统一标准，由于医疗及经济条件的限制，我国多数患者血液透析开始较晚。透析指征：①内生肌酐清除率<10ml/min;②BUN>28、6mmol/L，或SCr>707、2umol/L;③高钾血症;④代谢性酸中毒;⑤口中有尿毒症气味伴食欲丧失和恶心、呕吐等;⑥慢性充血性心力衰竭、肾性高血压或尿毒症性心包炎用一般治疗无效者;⑦出现尿毒症神经系统症状，如性格改变、不安腿综合征等。开始透析时机时同样需综合各项指标异常及临床症状来作出决定。 　　三、 急性药物或毒物中毒：凡能够通过透析膜清除的药物及毒物，即分子量小，不与组织蛋白结合，在体内分布较均匀均可采用透析治疗。应在服毒物后8~12h内进行，病情危重者可不必等待检查结果即可开始透析治疗。 　　四、 其它 疾病：严重水、电解质及酸解平衡紊乱，一般疗法难以奏效而血液透析有可能有效者。 　　禁忌症 　　随着血液透析技术的改进，血液透析已无绝对禁忌症，只有相对的禁忌症。 　　1、 休克或低血压者(收缩压<80mmHg) 　　2、 严重的心肌病变导致的肺水肿及心力衰竭; 　　3、 严重心律失常; 　　4、 有严重出血倾向或脑出血; 　　5、 晚期恶性肿瘤; 　　6、 极度衰竭、临终患者; 　　7、 精神病及不合作者或患者本人和家属拒绝透析者。
