ALD是一种临床异质性很强的遗传性代谢性疾病。表现在同一家系可有不同表型，同一患者不同时期表现也不同。根据ALD的发病年龄及临床表现分为7型：儿童脑型、青少年脑型、成人脑型、肾上腺脊髓神经病型(Adrenomyeloneuropathy， AMN)、Addison 型、无症状型和杂合子型。 诊断依赖于临床表现及以下指标的检测：(1)内分泌功能检测:对于肾上腺皮质功能不全患者，24h尿17-羟类固醇和17-酮类固醇排出减少；血浆促肾上腺皮质激素升高；促肾上腺皮质激素兴奋试验呈低反应或无反应。(2)血浆、皮肤成纤维细胞VLCFA测定: VLCFA增高是目前诊断的主要生化指标，见于几乎所有男性患者及80%女性携带者。VLCFA检测应用气相色谱法检查血浆、红细胞和培养的皮肤成纤维细胞中VLCFA的异常升高是诊断本病的特异方法，见于所有男性患者及85%的女性杂合子。VLCFA的升高水平与病情的严重程度无关。检测培养的羊膜细胞和绒毛膜细胞中VLCFA，可作为产前诊断。(3) 影像学表现CT表现为局限于脑白质的低密度蝶形病灶；MRI优于CT，能显示视觉、听觉传导通路和运动传导通路上的病灶。颅脑MRI具有特征性改变，MRI的表现可以领先或与ALD症状同时出现，并随着病情的发展而发展。表现为对称性位于双侧顶枕区白质长Tl长T2号，周边呈指状，胼胝体压部早期受累，呈“蝶翼状”，是ALD所特有的，其他脑白质病少见；ALD的一个显著特点是病变由后向前进展，逐一累及枕、顶、颞、额叶；可累及脑干皮质脊髓束，皮质下U形纤维免于受累；增强扫描病灶周边强化，提示处于活动期；晚期增强后无强化，多伴有脑萎缩。ALD的不同阶段在头部MRI上表现不同，可借此作为治疗转归和判断预后的指标。1HMRS 人脑的MRS技术是近十几年来开始应用于临床的。由于人体氢质子含量较多，具有较高灵敏度及空间分辨力，故1H-MRS应用最为广泛。在ALD中，1HMRS表现为Cho波峰显著增高，NAA波峰降低或消失，Lac波峰增高。 1HMRS对ALD的早期诊断有重要帮助，可以先通过MRI发现中枢神经系统的脱髓鞘病变，在MRI表现正常区代谢产物已有改变，MRI和1HMRS表现可以直接反映病变的严重程度。(4)病理检查脑组织、周围神经、肾上腺、直肠粘膜等处的病理检查发现细胞内含有板层状结构的胞浆包涵体可确诊本病。 A. 肾上腺皮质激素替代治疗：大部分患儿存在肾上腺皮质功能不全，因而皮质激素替代治疗常常是必需的，但是此治疗对神经系统病损无效，不能改善症状，也不能阻止病变恶化。主要适用于Addison患者，对于脑型患者未见明显改善。B. 考虑到脑型ALD的炎性脱髓鞘反应，有人试用环磷酰胺和免疫球蛋白治疗，但未见疗效。 洛伐他汀(Lovastatin)是一种3-HMG-CoA合成酶抑制剂，能够阻止星形细胞、小神经胶质细胞以及巨噬细胞释放炎性介质，加强VLCFA的-氧化作用，可使ALD患者皮肤成纤维细胞中的VLCFA水平降低。(3)骨髓移植：骨髓移植可以纠正VLCFA的代谢紊乱、重建酶的活性，改善临床症状，防止痴呆，以早期治疗效果更好。随着骨髓移植技术的提高和无症状ALD的早期检出，骨髓移植有一定的治疗前途。但骨髓移植本身有一定的病死率，且价格昂贵，一般认为 适应于起病早期的儿童型ALD。(4)基因治疗这是最有前途的治疗手段，但因技术问题有待进一步探讨。(5)造血干细胞治疗：最近，造血干细胞治疗也有用于ALD患者并获得了一定的疗效，目前认为宜用于早期患者，进展型患者可以作为候选治疗。但不推荐用于头颅MRI正常的无症状患者、单纯AMN型患者。
