5.常见过量表现有呼吸困难和心动过速。 5.12 西咪替丁的不良反应 1.较常见的有腹泻、恶心、呕吐、腹胀、便秘、口苦、口干、血清氨基转移酶轻度升高等，偶见严重肝炎、肝坏死、肝脂肪性变等。对肝硬化患者，可能诱发肝性脑病。突然停药，可能引起慢性消化性溃疡穿孔，估计为停用后回跳的高酸度所致。另有报道西咪替丁可致急性胰腺炎。 2.血液系统：西咪替丁对骨髓有一定的抑制作用，可出现中性粒细胞减少、全血细胞减少；也有报道出现血小板减少、粒细胞缺乏；仅有个案报道可出现自身免疫性溶血性贫血、再生障碍性贫血、嗜酸粒细胞增多。西咪替丁血液系统的不良反应多见于并发症严重之患者，或接受抗代谢的烃基类药物或其他导致粒细胞减少的治疗等。 3.神经/精神系统：（1）头晕、头痛、疲乏、嗜睡等较常见；少数患者可出现可逆性的意识混乱、定向力障碍、不安、感觉迟钝、语言含糊不清、局部抽搐或癫痫样发作、谵妄、抑郁、幻觉、锥体外系反应以及运动性多神经病等。出现神经毒性症状后，一般只需适当减少用药剂量即可消失，也可用拟胆堿药毒扁豆堿治疗。（2）在治疗酗酒的胃肠道并发症时，可出现震颤性谵妄，酷似戒酒综合征，应注意区分。（3）西咪替丁的神经精神系统不良反应主要发生在重症患者，老年患者、幼儿、肝肾功能不全者、有精神病史者、有脑部疾病者以及大剂量用药时也易发生。另外，假性甲状腺功能低下者可能对西咪替丁的神经毒作用更敏感。 4.内分泌/代谢系统：西咪替丁具有轻度抗雄性激素作用，可出现脂质代谢异常、高催乳素血血症、血浆睾睾酮水平下降和促性腺素水平增加、男性 *** 发育和 *** 胀痛以及女性溢乳等。 5.心血管系统：可出现心动过缓、面部潮红等。静脉注射时偶见血压骤降、房性早搏、心跳呼吸骤停等。 7.眼：可出现视神经病变。推测系西咪替丁具有锌螯合作用，使体内锌含量不足，从而引起视神经病变。另有出现眼肌 *** 报道。 8.皮肤：西咪替丁可抑制皮脂分泌，诱发剥脱性皮炎、皮肤干燥、皮脂缺乏性皮炎、脱发等；也可发生过敏反应（如皮疹、巨型荨麻疹）、史约综合征及中毒性表皮坏死溶解等。 9.肌肉骨骼：长期用药后可出现肌痉挛或肌痛。 10.致癌性：西咪替丁对鼠的长期毒性研究发现，良性Leydig细胞瘤的发生率较对照组高，但临床上未见此不良反应。 11.其他：（1）西咪替丁可降低胃内酸度，引起胃内微生物的滋生，在反胃的情况下可出现感染，应引起重视。（2）罕见发热、嗅觉减退等。 5.13 西咪替丁的用法用量 1.（1）十二指肠溃疡或病理性高分泌状态：每次300mg，每天4次，或800mg睡前一次服用。疗程一般为4～6周。治疗卓艾综合征时用量可达每天2g。（2）预防溃疡复发：睡前每次服用400mg。（3）反流性食管炎：每天800～1600mg，疗程为4～8周，必要时可延长4周。（4）反流性食管炎的对症治疗：出现烧灼感和（或）有反胃时可服用200mg；最大剂量为每次200mg，每天3次。疗程不得超过2周。 1.西咪替丁为肝药酶抑制剂，通过其咪唑环与细胞色素P450结合而降低药酶活性，同时也可减少肝血流。故西咪替丁与普萘洛尔合用时，可使后者血药浓度升高，休息时心率减慢；与苯妥英钠或其他乙内酰脲类合用时，可使后者的血药浓度升高，可能导致苯妥英钠中毒，必须合用时，应在5天后测定苯妥英钠的血药浓度以便调整剂量。 2.与环孢素合用时，可使后者的血药浓度增加。 3.与吗氯贝胺合用时，可使后者的血药浓度增加。 4.与茶堿合用时，可使后者的去甲基代谢清除率降低20%～30%，血药浓度升高。 5.与阿司匹林合用时，可导使阿司匹林的溶解度增高，吸收增加，作用增强。 6.与美沙酮合用时，可增加后者的血药浓度，有导致过量的危险。 7.与他克林合用时，可增加后者的血药浓度，有导致过量的危险。 8.与卡马西平合用时，可增加后者的血药浓度，有导致过量的危险。 9.西咪替丁可使维拉帕米（异搏定）的绝对生物利用度升高。由于维拉帕米可发生严重的不良反应，虽少见，但仍应引起注意。 10.与香豆素类抗凝药合用时，可使后者自体内排出率下降，凝血酶原时原时间进一步延长，从而导致出血倾向。两者合用时须密切注意病情变化，并调整抗凝药用量。 11.与利多卡因（胃肠道外用药）合用时，可使后者的血药浓度增加，从而增加其发生神经及心脏不良反应的危险。两者合用时需调整利多卡因剂量，并加强临床监护。 12.与咖啡因合用时，可延缓咖啡因的代谢，加强其作用并易出现毒性反应。胃溃疡患者本来忌用咖啡因，服用西咪替丁时更应禁用咖啡因及含咖啡因的饮料。 13.与苯二氮卓类药物（如地西泮、硝西泮、氟硝西泮、氯氮、咪达唑仑、三唑仑等）合用时，可抑制后者的肝内代谢，升高其血药浓度，加重其镇静及其他中枢神经抑制症状，并可发展为呼吸及循环衰竭。但是其中劳拉西泮、奥沙西泮与替马西泮似乎不受影响。 14.同时服用地高辛和奎尼丁的患者不宜再并用西咪替丁，因为西咪替丁可抑制奎尼丁的代谢，而后者可将地高辛从其结合部位置换出来，结果使奎尼丁和地高辛的血药浓度均升高。 15.与抗酸药（如氢氧化化铝、氧化化镁）合用时，可缓解十二指肠溃疡疼痛，但西咪替丁的吸收可能减少，故一般不提倡两者合用。如必须合用，应适当增加西咪替丁剂量。 16.与甲氧氯普氯普胺合用时，西咪替丁的血药浓度可降低。两者如需合用，应适当增加西咪替丁剂量。 17.由于硫糖铝需经胃酸水解后才能发挥作用，而西咪替丁抑制胃酸分泌，故两者合用时，硫糖铝的疗效可能降低。 Those Beautiful Flowers ---Book Review: “Camille” I read the Chinese version of “Camille” a few years ago. At that time I was deeply moved by the main character Marguerite Gautier. “Camille” or “The Lady of the Camellias” by Alexandre Dumas, fils, is the story of Marguerite Gautier, a young courtesan, or kept woman, in Paris in the mid 1800"s, and how she falls in love with a young man, Armand Duval, and then tries to escape from her questionable past. Unfortunately, it comes back to haunt her and she ends up returning to that life and dies painfully and alone, but with the knowledge that she was a noble woman at heart. When I first began to read the book, I did not care for Marguerite or her attitude or lifestyle, but as I got further into the narrative, I realized that her saucy attitude was a front to cover the lonely woman that she really was. She felt used, abused and unloved, until the gentle Armand Duval came into her life and showed her that he loved her as a person and not for what she could do for him. It must have taken great courage for Marguerite to leave the life she had lived for so long, knowing all along that it was probably too good to be true and would not last indefinitely. And it also showed that Marguerite really loved Armand Duval for she could even change herself for him. However, happiness didn"t last for long. When M. Duval, Armand"s father, came to her, pleading for her to leave Armand to save both Armand"s reputation and that of his younger innocent sister, Marguerite saw a way to become pure of heart, if not in body. She felt that it was her duty, because she loved Armand so much, to do this even though it meant giving up her own happiness and hurting Armand temporarily. She reluctantly returned to her former life, knowing that some day Armand would forgive her. Sadly, she died in debt and basically alone, except for her one female friend, Julie Duprat, who helped her during her illness. She had her journal sent to Armand after her death, explaining why she had made the choices she had. I think Dumas"s last few lines about Marguerite being the exception, not the rule were quite true, and I also agreed with his view that while her lifestyle could not be condoned, we as a society assume that all of these type of women are cold and heartless, while this may not always be the case. A person can make the wrong choices in life when they are young, and try to redeem themselves, but sometimes past situations prevent them from changing their lives, even though they desperately wish to do so. This applies to both men and women in many different types of circumstances: involvement in crime; drug or alcohol abuse; gambling; prostitution; financial problems; poor marriage choices; etc. And this is the fact, which exists in the whole society. As far as the other characters in the book, I think Marguerite was right in saying that no one truly cared about her, but only wanted something from her, the only exceptions being Armand and Julie Duprat. Of course, the Comte de G. and Comte de N. wanted her body and appearance. The Duke needed to “wake up and smell the coffee” and realize that she could never replace his dead daughter. If he truly cared, he could have helped her leave her lifestyle without “keeping” her himself. And lastly, Prudence was a blood-sucking leech who used Marguerite almost worse than the men. I also think she was jealous of the fact that Marguerite had so much more courage than herself and someone truly loved her. As we are normal persons, even we can see the hideousness of humanity that results in their fate of withering, we can at most ask quietly in our hearts: Where have those beautiful flowers gone? Where have they gone?

Those Beautiful Flowers 　　---Book Review: “Camille” 　　I read the Chinese version of “Camille” a few years ago. At that time I was deeply moved by the main character Marguerite Gautier. “Camille” or “The Lady of the Camellias” by Alexandre Dumas, fils, is the story of Marguerite Gautier, a young courtesan, or kept woman, in Paris in the mid 1800"s, and how she falls in love with a young man, Armand Duval, and then tries to escape from her questionable past. Unfortunately, it es back to haunt her and she ends up returning to that life and dies painfully and alone, but with the knowledge that she was a noble woman at heart. When I first began to read the book, I did not care for Marguerite or her attitude or lifestyle, but as I got further into the narrative, I realized that her saucy attitude was a front to cover the lonely woman that she really was. She felt used, abused and unloved, until the gentle Armand Duval came into her life and showed her that he loved her as a person and not for what she could do for him. It must have taken great courage for Marguerite to leave the life she had lived for so long, knowing all along that it was probably too good to be true and would not last indefinitely. And it also showed that Marguerite really loved Armand Duval for she could even change herself for him. 　　However, happiness didn"t last for long. When M. Duval, Armand"s father, came to her, pleading for her to leave Armand to save both Armand"s reputation and that of his younger innocent sister, Marguerite saw a way to bee pure of heart, if not in body. She felt that it was her duty, because she loved Armand so much, to do this even though it meant giving up her own happiness and hurting Armand temporarily. She reluctantly returned to her former life, knowing that some day Armand would five her. Sadly, she died in debt and basically alone, except for her one female friend, Julie Duprat, who helped her during her illness. She had her journal sent to Armand after her death, explaining why she had made the choices she had. I think Dumas"s last few lines about Marguerite being the exception, not the rule were quite true, and I also agreed with his view that while her lifestyle could not be condoned, we as a society assume that all of these type of women are cold and heartless, while this may not always be the case. A person can make the wrong choices in life when they are young, and try to redeem themselves, but sometimes past situations prevent them from changing their lives, even though they desperately wish to do so. This applies to both men and women in many different types of circumstances: involvement in crime; drug or alcohol abuse; gambling; prostitution; financial problems; poor marriage choices; etc. And this is the fact, which exists in the whole society. 　　As far as the other characters in the book, I think Marguerite was right in saying that no one truly cared about her, but only wanted something from her, the only exceptions being Armand and Julie Duprat. Of course, the Comte de G. and Comte de N. wanted her body and appearance. The Duke needed to “wake up and *** ell the coffee” and realize that she could never replace his dead daughter. If he truly cared, he could have helped her leave her lifestyle without “keeping” her himself. And lastly, Prudence was a blood-sucking leech who used Marguerite almost worse than the men. I also think she was jealous of the fact that Marguerite had so much more courage than herself and someone truly loved her. 　　Last morning, when tiding my bookshelf, I took this book out of the shelf, and a dried flower flew away from the book. As we are normal persons, even we can see the hideousness of humanity that results in their fate of withering, we can at most ask quietly in our hearts: Where have those beautiful flowers gone?
