　　靶向治疗为肿瘤治疗带来了新的革命，将成为本世纪肿瘤治疗中最具潜力、最有希望的疗法。下面是我推荐给大家的，供大家参考。 　　一、EGFR突变 　　EGFR***英语：epidermal growth factor receptor，简称为EGFR、ErbB-1或HER1***是表皮生长因子受体***HER***家族成员之一。该家族包括HER1***erbB1，EGFR***、HER2***erbB2，NEU***、HER3***erbB3***及HER4***erbB4***。HER家族在细胞生理过程中发挥重要的调节作用。 　　EGFR基因在亚裔腺癌人群中的突变频率是10%-40%，常见突变位点：19外显子缺失突变，21外显子L858R点突变及18与20外显子突变等。 　　一代易瑞沙、特罗凯、凯美纳靶向的出世，创造了肺癌治疗史上的奇迹。后续抑制作用较强的二代不可逆抑制剂阿法替尼也进入大家视野，阿法替尼是靶向EGFR及HER2的双靶点抑制剂，疗效肯定，但国人的不良反应相对多些。而一代二代药物治疗在一年左右的时间会出现不同程度的耐药，在对于耐药机制的研究中发现了T790M的二次突变，针对于此的三代EGFR-TKI抑制剂AZD9291也因此普及，形成了目前三代EGR-TKI的治疗阶梯。我们也要认识到国内上市的靶向药物只有一代的易瑞沙，特罗凯，凯美纳，二代药物阿法替尼即将上市，三代药物也只有临床试验，基于药物未在中国上市，病情进展后可以进行化疗。 　　二、ALK融合或重排 　　ALK融合或重排在肺腺癌中占5%-10%，其常见的融合方式是EML4-ALK。其靶向的一代药物是国内已有的克唑替尼，可产生74%的有效率。而长期使用的患者也会在中位1-2年时间出现耐药，即发生ALK守门基因的突变导致继发耐药，从而促发了二代甚至是三代药物的出现，也形成了ALK治疗的三阶梯，但国内上市的ALK靶向药物只有克唑替尼一种，化疗仍是ALK耐药后的选择。 　　三、ROS1的融合 　　ROS1的融合在肺腺癌中突变率较低，1.2%--1.7%，常见于年轻、非吸菸、腺癌、高级别组织病理型别患者。这一基因融合与ALK类似，具有明确的肿瘤形成驱动性及较好的临床疗效。因此2017.2版NCCN指南中首次将ROS1提到肺腺癌的常规检测中，制定了该类患者特定的临床治疗路径。 　　对于ROS1重排阳性的患者，一线初试治疗推荐克唑替尼用药，有效率可达到72% ，耐药时间19.2 月，病情进展后可以进行化疗或PD-1的免疫治疗。 　　肺癌的免疫治疗 　　1、抗体靶向的免疫治疗在很多恶性肿瘤上已获得早期成功，在晚期肺癌和其它肿瘤的治疗中已经起到了很大的作用。 　　2、一些靶向PD-1受体的单克隆抗体***nivolumab, pembrolizumab*** 或其配体PD-L1 ***atezolizumab, durvalumab, avelumab***已应用于临床。这些药物的早期临床试验已证实在经治疗的晚期NSCLC患者中，14-20%表现出迅速且持久的反应，且生存率的改善显著。最主要的副作用是无力、疲劳、丧失食欲、恶心及腹泻。 　　3、抗PD-1或抗PD-L1药物的临床疗效及安全性被4个随机临床试验支援，其中有代表性的2个是在经基于铂类化疗治疗的患者中比较nivolumab与多西他赛治疗效果--CheckMate 017试验和CheckMate 057试验。 　　4、检查点抑制剂的获益主要限于PD-L1阳性肿瘤人群。Pembrolizumab在PD-L1表达细胞数≥50%的情况下，已被FDA批准作为二线及以上线的治疗。默克公司宣布一项达到终点的大型研究表明，在PD-L1表达细胞数≥50%的情况下，pembrolizumab与化疗相比，PFS和OS获益明显。在一项包含所有NSCLC组织学亚型的II期试验中，atezolizumab与多西他赛相比，表现出明显获益。 　　5、整体的资料表明PD-1抑制剂倾向于在经过标准化疗的肺鳞癌中作为二线治疗，在非肺鳞癌中至少需要满足PD-L1阳性的条件。 　　6、在之前未经治疗的患者中，抗PD-1或抗PD-L1抑制剂的早期试验的结果喜人--在PD-L1阳性患者中1年生存期超过70%。几项试验正在比较这些药物联合铂类治疗方案作为一线治疗的效果。 　　肺癌的症状 　　1、咯血 　　气管，支气管和肺组织出血，经口腔咯出称为咯血，咯血量的多少视病因或病变的性质而异。 　　2、呼吸异常 　　人呼吸频率成人为16~20次/min，与心脏搏动次数的比例为1：4。 　　3、痰有恶臭味 　　痰的气味呈恶臭味时，肺部炎症伴有厌氧菌感染。见于肺脓肿、支气管扩张及肺的恶性肿瘤晚期等肺部疾病。 　　4、鼠尾征 　　鼠尾征指支气管肺癌沿支气管的纵轴浸润蔓延，使支气管呈尾巴状狭窄且不规则的缺损。

最近又在门诊看了几个年轻的肺腺癌患者，都是女性，比较幸运的是都是早期肿瘤，不需要后续的治疗。近十年，肺癌的病理类型发生了很大的变化，与以前不同，如今肺腺癌占据发病率第一的位置，占所有肺癌人群的40－50%，早中期肺腺癌仍然是根治性手术治疗为主，而IV期肺癌病人一般没有手术治疗机会，治疗手段包括放化疗、靶向治疗和免疫治疗。IV期肺腺癌病人容易出现其他脏器转移，比如肝转移、脑转移、骨转移和肾上腺转移。总体来说，预后比较差，治愈的机会微乎其微。IV期肺腺癌病人能活多久？ 很多病人和家属总会喜欢问“能活多久”这样的问题，实际上，对于个体来说，很难回答具体能活多久，同样的晚期肺腺癌病人，即便同样的治疗方法，效果会千差万别，寿命会差异很大。晚期肺癌病人的生存期与很多因素有关，比如病人的年龄、有没有基础疾病、身体状况怎样、是单发转移还是广泛多脏器转移、有没有进行规范治疗、有没有敏感基因突变、治疗效果如何，这些都决定了不同的病人有不同的生存期。根据第八版肺癌分期对近万例的患者随访结果，IV肺癌病人五年生存率在10%以内，1年生存率为30%-40%，即便在传统放化疗的情况下大部分病人生存期在1－3年。但肺腺癌不同于肺鳞癌的地方是有针对性的靶向药可以选择，所以近十年晚期肺腺癌病人的总体生存期在延长。中国有EGFR突变的肺腺癌人群在50%－55%，这类病人很幸运的能够进行靶向治疗。还有少数肺腺癌病人有ALK基因突变，ROS1基因突变，都可以选择相应的靶向药物。目前，针对EGFR基因的靶向药物大幅度降价，且在医保之内，尤其是一代靶向药吉非替尼，国产仿制药才四百多一盒，还可以报销，这个价格几乎人人能够用得起。在门诊随访的晚期肺腺癌病人中，最长的达到11年，这个病人发现时有骨转移、肝转移，口服易瑞沙后数月病灶就完全消失，肺部残留一些纤维灶，迄今为止这个病人仍不敢停药。其他像口服五六年靶向药的人不在少数。8月31日，国家药品监督管理局批准奥希替尼（泰瑞沙）用于EGFR基因突变型晚期非小细胞肺癌的一线治疗，这是一个具有重大意义的好消息，研究显示，一线使用奥希替尼治疗EGFR阳性肺癌，耐药后使用放化疗等综合治疗方式，病人的中位总生存期是41.4个月，达到了一代靶向药的2倍之多。也就是说，IV期肺腺癌病人使用奥西替尼，一半的病人寿命能达到三年半，这个在传统治疗中是不可想象的。此外，还有针对其他靶点的靶向药和免疫治疗药物在IV期肺腺癌病人中同样发挥了巨大作用，比如新一代ALK抑制剂—阿来替尼(安圣莎)创造了34.8个月无进展生存期的神话，同时还能有效治疗和预防脑转移。KEYNOTE-189试验研究了K药联合化疗治疗晚期非鳞非小细胞肺癌，联合治疗组高达69.2%，而单独化疗组只有49.4%，免疫联合化疗降低了50%的死亡风险。新药、新技术层出不穷，只要有药可用，IV期肺腺癌病人生存时间就可以不断延长。 IV期肺癌大概能活多久？几乎每天都有患者问类似问题。换位思考，完全能理解患者心情。 IV肺癌的预后要看肺癌的病理类型、是否有基因突变、单发转移还是多发转移、是否积极治疗、患者对药物的反应性等很多情况。 比如说，EGFR基因突变的IV期肺腺癌，在十年前平均十个月生存期，因为那个时候没有靶向药物。得益于第一代靶向药物的应用，三年前这部分患者平均生存期达到两年。而现在如果积极治疗，EGFR基因突变的IV肺腺癌平均能有四十个月以上的生存期，因为有第三代靶向药的应用。 ALK基因突变的肺腺癌，现在平均生存期已经在五十个月左右。相信再过三年，又是一个新的进步。 这几年，针对一些少见突变的新药也在陆陆续续被研发，例如NTRK和20插入突变；过去认为无法用靶向药物治疗的KRAS突变，现在也有对应的靶向药物被研发出来。 最后，目前没有任何基因突变的肺癌，也可以接受免疫治疗和化疗。9月10即将公布在世界肺癌大会的数据显示：免疫治疗联合多靶点治疗可以让77％的患者病灶至少缩小30％。 所以，坚持，会有新希望 晚期肺腺癌的生存差异非常大，很难给出一个大概的范围。 一、有EGFR基因突变的患者 这部分患者因为有靶向治疗的机会，所以，一代靶向药物就能把疾病控制1年左右，在出现肿瘤耐药的时候，如果属于耐药基因突变的类型，三代靶向药物能够把病情再控制1年左右，再次出现耐药的情况下，化疗或化疗联合靶向抗血管仍然可以把病情控制半年到1年。 二、有ALK基因突变的患者 这类患者虽然占比较少，但是一旦检测有ALK基因突变，靶向抗ALK的药物能够把晚期患者的生存期大大延长，目前二代靶向抗ALK的药物，最长的观察数据已经达到了7年，仍有部分患者生存。 三、没有敏感基因突变，但PD-L1表达超过50%的患者 这类患者大多没有合适的靶向药物可用，但却是免疫治疗（免疫检查点抑制剂）的适应症，目前一线应用免疫治疗的患者，随访数据刚过5年，但仍有约30%的患者生存。 四、单纯化疗的患者 如果没有靶向抗肿瘤的机会，免疫治疗也疗效欠佳，单纯化疗的患者，生存期基本上在半年到1年。 总之，虽然都是晚期肺腺癌，但生存差异巨大，很难给出一个生存区间预判，需要根据具体患者的具体情况，才能有一个大概的预测。上周国家食药监局刚刚批准奥西替尼泰瑞沙也就是9291为一线用药，在国内特别不抽烟女性肺腺癌患者egfr突变概率很高，根据9291的临床数据显示，其生存期是18个月。也就是说，肺腺癌晚期患者服用第三代靶向药正常情况下的平均生存期是18个月。具体到个人头上肯定是有差距的，我们的病友群里一位上海的病人吃一代超过十年，一位南京的病人吃了六年，刚刚换了9291，个体差异很大的。 至于alk突变，更有钻石突变之说，所谓钻石突变，就是说这种突变概率相对来说很稀有，也说明这种突变生存期超长，临床数据显示一代用克唑替尼是10个月，二代阿来替尼19个月，布加替尼更是达到了26个月，而且三代劳拉替尼也已经上市了。 其次突变概率也较高的ros1靶点用药方案和alk非常接近。 至于其它突变概率更小的靶点比如去年炒的非常火热的ntrk所用的拉罗替尼生存期更长，有效率更高，her2.cmet.ret等也有相应药物，但效果副作用都有差距了。 至于没有突变不能靶向治疗的，如果pd1表达高于五十，经济条件允许，可以尝试免疫疗法，免疫疗法的主要问题是有效率相对低，只有20%左右，如何提高有效率是目前最大的问题，一旦有效，其生存期会比其它治疗方案更乐观，经济条件不允许则只能化疗放疗，化疗大家都知道，副作用大有效率低，这种情况的生存期生存质量也没法和靶向治疗相比了。 总之，肺腺癌的治疗关键在于基因检测，效果如何或者生存期多久看基因突变类型和用药方案，但是就算靶点一样药一样，也同样还存在个体差异。只能说，科学治疗疾病，理性对待病情吧！ 肺癌iv期还能活多久？看下真实的临床数据 肺癌四期还能活一年到三年的情况，生存的时间在于肺癌疾病的类型及治疗效果决定的。 如果患者相关检查为中央型肺癌可以采取纤维支气管镜活检病理确诊，如果为周围型肺癌在CT引导下行肺穿刺活检病理确诊，结合病理类型采取化疗、放疗治疗，其次非小细胞肺癌可以采取靶向药物治疗，如果患者治疗效果好的情况下可以活三五年的。 患者都是被一帮无知的饭桶治死的！！！无知至极！！！！给患者 健康 必须只有做到:一排除患者体内毒素垃圾，二增强患者自身免疫力。做不到这两点谈治疗无异于谋财害命。。现在的医疗手段步入了误区，无知的治疗只会雪上加霜，肿瘤是病的源吗？肯定不是。化疗放疗杀癌细胞，不杀好细胞吗？？手术化疗只会再一次增加患者体内毒素垃圾，再一次降低免疫力。国家养了一帮无知至极的废物，饭桶！！！
