目录 1 拼音 2 英文参考 3 概述 4 疾病名称 5 英文名称 6 地方性斑疹伤寒的别名 7 分类 8 ICD号 9 流行病学 9.1 传染源 9.2 传播途径 9.3 人群易感性 9.4 流行特征 10 病因 11 发病机制 12 地方性斑疹伤寒的临床表现 12.1 潜伏期 12.2 症状和体征 13 地方性斑疹伤寒的并发症 14 实验室检查 14.1 血象 14.2 血清学检查 14.3 动物接种 15 辅助检查 16 诊断 17 鉴别诊断 18 地方性斑疹伤寒的治疗 18.1 一般治疗 18.2 病原治疗 18.3 对症治疗 18.4 中医治疗 19 预后 20 地方性斑疹伤寒的预防 21 相关药品 22 相关检查 附： 1 地方性斑疹伤寒相关药物 1 拼音 dì fāng xìng bān zhěn shāng hán 2 英文参考 endemic typhus 3 概述 地方性斑疹伤寒（endemic typhus）是由莫氏立克次体引起，通过鼠蚤传播的急性传染病。又称蚤传或鼠型斑疹伤寒（murine typhus）。潜伏期为1～2周，临床表现与流行性斑疹伤寒相似，但症状轻，病程短，皮疹很少呈出血性，预后好，病死率低。 4 疾病名称 地方性斑疹伤寒 5 英文名称 endemic typhus 6 地方性斑疹伤寒的别名 endemic typhus fever；fleabrone/murine typhus；鼠斑疹伤寒；鼠型斑疹伤寒；蚤传或鼠型斑疹伤寒 7 分类 感染内科 > 立克次体感染 > 斑疹伤寒 皮肤科 > 立克次体和衣原体感染疾病 > 立克次体病 8 ICD号 A75 9 流行病学 地方型斑疹伤寒散发于全球，多见于热带和亚热带，属自然疫源性疾病。本病以晚夏和秋季谷物收割时发生者较多，并可与流行型斑疹伤寒同时存在于某些地区。国内以河南、河北、云南、山东、北京市、辽宁等的病例较多，1982～1984年间有多篇文献报道。 9.1 传染源 地方性斑疹伤寒的主要传染源是家鼠，一般情况是以鼠→鼠蚤→鼠的循环形成在鼠间传播。鼠感染后大多并不死亡，而鼠蚤只在鼠死后才离开鼠体择人吮血使人感染。莫氏立克次体可在虱体内生长繁殖，病人也有可能作为本病的传染源。 9.2 传播途径 鼠蚤通过吸吮病鼠血而致感染，病原体进入鼠蚤肠道内繁殖。当鼠蚤叮咬人时，同时排出含有病原体的粪便和呕吐物，病原体可经抓伤破损的皮肤侵入人体，或蚤被打扁压碎逸出的病原体可通过同一途径侵入人体。干蚤粪内的病原体偶可成为气溶胶经呼吸道或眼结膜等感染人。螨、蜱等节肢动物亦可带本病原体，而成为传病媒介的可能。 9.3 人群易感性 人群对本病普遍易感。感染后可获得持久性免疫力，并与流行性斑疹伤寒有交叉免疫。隐性感染率高，在流行区的健康人群中，50%～80%可测得特异性抗体，感染后及病后均可产生强而持久的免疫力，且对普氏立克次体感染亦有相当的免疫力。据报告，小学生及青壮年发病者居多。据新加坡报道，近年来本病发病率有所增加，20～40岁者多见，男性高于女性，种族是印尼人多于马来人，中国人少于前二者。 9.4 流行特征 地方性斑疹伤寒的流行呈散发性，夏、秋季多发，多见于温带及亚热带地区，国内以西南、华北诸省较多。 10 病因 地方性斑疹伤寒的病原体为莫氏立克次体，其形态、染色特点、生化反应、培养条件及抵抗力等均与普氏立克次体相似，但很少呈长链排列。而在实验动物病损以及抗原性方面有所不同。两者各含3/4种特异性颗粒性抗原，和1/4组特异性可溶性抗原；后者耐热，为两者所共有，故可产生交叉反应。不耐热的颗粒性抗原则各具特异性，可藉补结试验而相互区别。莫氏立克次体所致的豚鼠阴囊反应远较普氏立克次体所致者为明显，对小鼠和大鼠的致病性也较强。病原体接种于小鼠腹腔后可引起腹膜炎、立克次体血症，并在各脏器内查见病原体。 1.莫氏立克次体感染雄性豚鼠后，豚鼠除发热外阴囊高度肿胀，睾丸明显肿大，由于莫氏立克次体在睾丸鞘膜的浆膜细胞中快速繁殖，故在鞘膜渗出液涂片中可查见大量立克次体，称豚鼠阴囊现象；而普氏立克次体仅引起轻度的阴囊反应。 2.莫氏立克次体感染大白鼠可使其发热或致死，亦可在其脑内存活数月，故可用以保菌及传代；而普氏立克次体仅使大白鼠形成隐性感染。 3.用本病原体对小白鼠进行腹腔接种可引起致死性腹膜炎；而普氏立克次体则不能。 4.莫氏立克次体与普氏立克次体有共同的具有组特异性的耐热性可溶性抗原，故二者可有交叉免疫反应。各自又有不同的颗粒抗原，可借以通过凝集试验、补体结合试验及免疫荧光试验等与流行性斑疹伤寒相鉴别。莫氏立克次体寄生于鼠蚤肠壁细胞内，不影响鼠蚤生命；而普氏立克次体可使被感染的虱致死。 11 发病机制 地方性斑疹伤寒发病机制与流行性斑疹伤寒基本相似，但血管病变较轻，小血管、毛细血管血栓形成较少见。 12 地方性斑疹伤寒的临床表现 12.1 潜伏期 6～16天，多为12天。 12.2 症状和体征 与流行性斑疹伤寒相似，但中枢神经系统病状较轻。瘀点样皮疹少见。其特点： （1）起病多急骤，部分患者可有1～2天前驱症状如疲乏、食欲缺乏、头痛等。 （2）发热呈稽留或弛张热型，最高体温达38～40℃，伴有显著头痛，全身酸痛，关节痛，眼结膜充血等，热程一般9～14天，长者可达20天，热多渐退。 （3）皮疹较少，约60%～80%患者有皮疹，多在4～7天出现。初见于胸腹部，24h遍及全身，以躯干、四肢为多，面、颈、足底、手掌少见或无皮疹。皮疹形态多为充血性斑丘疹，边缘不整，大小不等，直径约1～5mm，颜色常由鲜红转为暗红，少数病例可见点状出血疹。持续数日皮疹消退，一般不留痕迹。中枢神经系统症状较轻，多数仅有头痛，很少有意识障碍及脑膜 *** 征等。中枢神经系症状除头痛、头晕、失眠、听力减退、烦躁不安等外，脑膜 *** 征、谵妄、昏迷、大小便失禁等均属偶见。 （4）其他尚可伴有咳嗽、恶心、呕吐、腹泻、轻度黄疸及脾脏肿大等。并发症以支气管炎多见，偶见支气管肺炎。咳嗽见于过半数病例，肺底偶闻罗音，部分患者诉咽痛和胸痛。大多有便秘，恶心、呕吐、腹痛等也有所见。黄疸在某些系列中的发生率较高，但均属轻度；脾肿大见于过半数病例，肝肿大者较少。心肌很少受累，偶可出现心动过缓。并发症以支气管炎最多见，支气管肺炎偶有发生。其他并发症有肾功能衰竭。 13 地方性斑疹伤寒的并发症 少数病例病情严重,可并发多脏器功能衰竭。 14 实验室检查 14.1 血象 大多数患者白细胞计数为正常。明显增多或减少者罕见。 14.2 血清学检查 （1）外斐反应中OX19呈阳性反应，效价较流行性斑疹伤寒为低，阳性反应出现于5～17病日，平均于11～15天。患者血清也可与变形杆菌OX19株发生凝集反应，效价为1∶160～1∶640，较流行性斑疹伤寒为低；阳性反应出现于第5～17病日，平均为第11～15病日。外斐试验虽然敏感，但特异性差，不可用以与流行性斑疹伤寒相区别。较为敏感和特异的试验包括间接免疫荧光抗体检测、乳胶凝集试验、补体结合试验、固相免疫测定等，所用抗原为特异性莫氏立克次体抗原。间接荧光抗体效价在发病后1周内升高者达50%，15天内升高者几乎达100%。有条件单位可用PCR方法检测血标本中的莫氏立克次体特异性核酸。 （2）以莫氏立克次体为抗原与患者血清作凝集试验、补体结合试验以及间接免疫荧光试验可与流行性斑疹伤寒相鉴别。 （3）部分患者可有一过性血清谷丙转氨酶升高。 （4）DNA探针杂交与PCR基因扩增技术联合，检测患者血中立克次体DNA同样可用于本病的早期诊断。 14.3 动物接种 将患者血液注入雄性豚鼠的腹腔，动物一般于接种后5～7天开始发热，阴囊因睾丸鞘膜炎而肿胀，鞘膜渗出液涂片可见肿胀的细胞质内有大量的病原体。需要注意一般实验室不宜进行豚鼠阴囊反应试验，以免感染在动物间扩散和实验室工作人员受染。 15 辅助检查 心电图可显示低电压，STT改变等。胸部X线检查常见肺部间质感染征象。 16 诊断 地方性斑疹伤寒诊断要点： 1.居住或工作环境有鼠，或有本病发生。 2.临床表现如发热、皮疹等特征与流行性斑疹伤寒相似，并且较轻。 3.血清学诊断有阳性结果。 根据以上几点不难作出诊断。 17 鉴别诊断 地方性斑疹伤寒主要与流行性斑疹伤寒鉴别，其病原体为普氏立克次体，传染源为患者，以体虱为媒介，多发于冬春季，呈流行性。症状与地方斑疹伤寒相似，但病情较重，皮疹多，出血性多见，神经系统症状明显，普氏立克次体凝集反应阳性。本病还需与流感、恙虫病、钩端螺旋体病等区别。 18 地方性斑疹伤寒的治疗 与流行型斑疹伤寒基本相同。国内某一系列（1985年）报道186例，101例以多西环素200mg顿服治疗；85例则采用四环素每日2g（4次分服），疗程5～7日。结果2日内退热者多西环素组为77%，四环素组为26%，两者间有显著差异（P＜0.05）。多西环素组的复发率为3.9%，四环素组为2.4%。国内也有认为除确诊早或有合并症者可加用多西环素外，其他病例可不必使用抗生素。 18.1 一般治疗 患者入院后先更衣、灭虱、卧床休息。保持口腔和皮肤清洁。危重患者要勤翻身防止并发症。供给富有营养易消化的饮食，补充大量的维生素B、C及足够的水分和电解质。 18.2 病原治疗 氯霉素、四环素族药物对本病皆有特效。一般于用药后十余小时症状开始减轻，2～3天内完全退热。氯霉素1.5～2g/d，分3～4次口服，退热后用量酌减，继续服3天，或延长至5～7天，以防近期内复发。多西环素0.2～0.3g顿服，必要时2～4天再服1剂。临床实践中氯霉素疗效虽好，因其副作用突出，已不作首选。而多西环素则应用较多，治疗简单，副作用少，效果满意。近来有用红霉素、氟喹酮类药物（如诺氟沙星、依诺沙星、环丙沙星）及米诺环素（Minocyciline）等治病本病也取得较好的效果。 18.3 对症治疗 高热以物理降温为主，必要时可给小剂量解热镇痛药。毒血症症状严重者可给予肾上腺皮质激素，有低血容量倾向或休克时按感染性休克处理。对有心功不全者要注意减轻心脏负荷，可用强心药如毛花苷C或毒毛花苷K等。头痛可给止痛药。对有精神症状者可给予地塞米松等治疗。 18.4 中医治疗 祖国医学认为本病属于瘟期常以辛凉解表、清热解毒为主；极期则以养阴清热为主，对缓解症状有一定效果。 19 预后 地方性斑疹伤寒大多预后良好，多在发病第2周恢复。但老年患者或未经治疗患者，感染后可陷于极度衰弱，恢复期延长。经有效抗菌药物治疗后痊愈。近年来发生暴发流行，仅极少数严重病例发生多器官功能衰竭死亡。 20 地方性斑疹伤寒的预防 1.主要是灭鼠、灭蚤，对患者及早隔离治疗。 2.本病多散发，故多不用预防注射疫苗。如有暴发流行，对高危人群应进行疫苗接种，可用普氏立克次体株灭活疫苗。 21 相关药品 氯霉素、四环素、多西环素、红霉素、诺氟沙星、依诺沙星、环丙沙星、米诺环素、毒毛花苷K、地塞米松 22 相关检查 补体结合试验、白细胞计数、外斐反应、血清谷丙转氨酶 地方性斑疹伤寒相关药物 棕榈氯霉素（B型）片 ．立克次体感染，可用于Q热、落矶山斑点热、地方性斑疹伤寒等的治疗。【用法用量】口服。成人一日1.5... 氯霉素胶囊 ．立克次体感染，可用于Q热、落矶山斑点热、地方性斑疹伤寒等的治疗。【用法用量】口服。成人一日1.5... 棕榈氯霉素（B型）颗粒 ．立克次体感染，可用于Q热、落矶山斑点热、地方性斑疹伤寒等的治疗。【用法用量】口服。成人一日1.5... 氯霉素胶囊 ．立克次体感染，可用于Q热、落矶山斑点热、地方性斑疹伤寒等的治疗。【用法用量】口服。成人一日1.5... 棕榈氯霉素（B型）颗粒
