您好，按照欧美人的习惯，total revenue and gains 中的 total 是针对 total revenue 和 total gains 但不是 total（revenue and gains），也就是 revenue 和 gains 视为两个独立的整体，total 变成了共享的形容词，而不是 revenue and gains 为一个整体或是“一套”，但这种用法通常只限于只有1个形容词，没有其他的修饰词。
假如 total 只针对 revenue 而不适用在 gains，通常会采取两种方式：

这里的 red 是指 pants，还是指 pants，socks 和 shirt，就比较难区分了，此时通常会按照常理判断，假如裤子可以是红色的，那袜子和衬衫是红色也就不奇怪了。就会被看做是 red pants, socks and shirt = red pants, red socks, and red shirt
但假如只有2个是红色的，而另一个不是，通常也是按照上面的方法：
1）使用（逗号）分开来，Give me the red pants and socks, and the shirt.
2）顺序对调：Give me the shirt, and the red pants and socks.
您会发现 and 的重复使用，和定冠词的重复使用。
英语语法中并没有对这种情况做出限制或是规则，基本上都是按照语意和读的人的理解能力了。但为了避免混淆，也会尝试重复修饰词，或是使用上面的两种办法。
