50/60ml “PLASTIPAK”设置注 射得普利麻。得普利麻亦可预先与阿芬太尼注射液（500mg/ml）按20:1至50:1的容积比混 合。混合物应无菌配制，并在配制后6小时内使用。为了减轻注射初期的疼痛，用于麻醉诱导时，可将得普利麻在塑料注射器中按不超过20:1的比例与0.5%或1%的利多卡因注射液混合使用，在注射前即刻配制。得普利麻丙泊酚稀释及和其他药物合并给药（参见 其它注意事项 TCI系统注射得普利麻用于成年人的全麻诱导及维持需受到 限制。不建议用于加强监护病人的镇静或清醒镇静或用于儿童。得普利麻可以借助目标控制滴注系统给药以达到成年患者麻醉诱导及维持。该系统允许麻醉师通过设定并调节血液中丙泊酚的目标浓度以达到并控制所需的诱导速度及麻醉深度。得普利麻只适用于“DIPRIFUSOR” TCI系统。该系统只 有在识别了得普利麻丙泊酚预充注射器上的电子识别标签才运行。该“DIPRIFUSOR” TCI系统根据所识别的得普利麻浓度自动调节静注速度。使用者必须熟悉注射泵的用户菜单和用TCI系统给 得普利麻 和正确使用识别系统，以上这些资料在 捷利康公司的“DIPRIFUSOR”培训手册中收载。以下所列为丙泊酚目标浓度指导。从病人间的药代动力学和药效学的个体差异的角度看，对于术前已给药和未给药的患者丙泊酚目标浓度应根据患者的反应逐渐增加以达到所需麻醉深度。对于55岁以下成年患者麻醉诱导时一般需要丙泊酚目标浓度4～8mg/ml。对于术前已给药的患者建议初始目标浓度为4mg/ml，对术前未给药患者建议初始目标浓度为6mg/ml。此目标浓度情况下的诱导时间一般为60～120秒。高目标浓度可加速麻醉诱导但同时可能伴随更显著的血流动力学及呼吸抑制。对于55岁以上和ASA3级和4级患者需较低的初始目标浓度。随后目标浓度每隔 1分钟增0.5～1.0mg/ml以达到逐步麻醉诱导。一般还需辅助镇痛药，并且维持麻醉所需的目标浓度可随辅助用药量而改变。一般情况下，得普利麻丙泊酚目标浓度在3～6mg/ml之间可维持满意的麻醉效果。预苏醒时的丙泊酚浓度一般为1.0～2.0mg/ml，并可因维持期间的镇痛药剂量而异。【不良反应】全身性麻醉诱导通常是平稳的，极少出现兴奋。在麻醉诱导期间，依剂量、使用的术前用药和其它药物而定，可能发生低血压和短暂性呼吸暂停。偶尔，静脉输液和维持麻醉期间，需要较低的得普利麻的给药速率，以纠正低血压。在复苏期间，只有少部分病人出现恶心、呕吐和头痛。在加强监护病人中，当得普利麻剂量超过每小时4mg/kg时有极其罕见的横纹肌溶解的报告。在麻醉诱导、维持及复苏期，惊厥和角弓反张的癫痫样运动，已偶见报道。给予得普利麻后，发生包括血管水肿、支气管痉挛、红斑和低血压的过敏症，但较罕见。肺部水肿亦曾有报告。手术后发热已见报道。与其它麻醉药一样，可能出现性欲兴奋。在延长得普利麻给药后，偶见尿变色的报道。局部性在得普利麻麻醉的诱导期可能出现局部疼痛，可通过合用利多卡因（见用法用量）或通过使用前臂较粗的静脉及肘前窝的方法来减轻，血栓形成和静脉炎罕见。事故性临床外渗和动物试验表明，得普利麻的组织反应极小。给动物动脉内注射，并不诱导局部组织反应。【禁忌症】已知对得普利麻过敏的病人禁用。【注意事项】得普利麻应该由受过训练的麻醉医师或加强监护病房医生来给药。用药期间应保持呼吸道畅通，备有人工通气和供氧及其它复苏设备。得普利麻丙泊酚不应由外科医师或诊断性手术医师给药。当得普利麻用于手术及诊断时的清醒镇静，应密切观察低血压、气路阻塞及氧气稀少的早期迹象。病人全身麻醉后，必须保证完全苏醒后方能出院。很罕见应用得普利麻时发生术后一段时间无意识，并有时伴有肌肉张力增加。这种情况发生之前，有时经过一段时间的复苏期，有时没有经过。虽然患者可以自行苏醒，但无意识的患者仍需看护。对于心脏、呼吸道、肾或肝脏损害的病人或者循环血容量减少及衰弱的病人，使用得普利麻与其它静脉麻醉药一样应该谨慎。得普利麻缺乏解除迷走神经作用的活性，并且已经与心动过缓（偶然较重）和心搏停止的报告联系在一起。在麻醉诱导前或麻醉维持期间，尤其是在迷走神经紧张性似乎占优势或当得普利麻与可能会引起心动过缓的其它药物合用时，应该考虑静脉给予抗胆碱能药物。当对癫痫病人使用得普利麻时，可能有惊厥的危险。脂肪代谢紊乱的病人和必须小心使用脂肪乳的其它情况下，应用得普利麻要适当小心。认为有脂肪超载特殊危险的病人，给予得普利麻丙泊酚时，建议监测血脂水平。如果监测表明，脂肪正在被不适当地从机体清除，那么得普利麻给药量应该相应调整。如果病人正在同时接受静注脂肪乳，则用量应该减少，因为要考虑到得普利麻制剂输注的脂肪量，即1.0毫升的得普利麻含约0.1克的脂肪。其它注意事项得普利麻不含抗微生物的防腐剂，并且支持微生物的生长。当抽吸得普利麻 时，必须严格无菌操作，在打开安瓶或小瓶启封后立即使用。在整个输注期 间，必须保持得普利麻和输注装置无菌。从得普利麻输注线上加任何药物或液体，必须在紧靠静脉置管的部位加入，得普利麻一定不能通过微生物滤器来给药。得普利麻和含得普利麻的任何注射器，都应一次性使用，根据已建立的其它脂肪乳剂的准则，单次输注得普利麻必须不超过12小时，在输注结束时或达12小时的时候，无论那种情况，得普利麻的贮器和输注线必须丢弃或替换。使用/处理注意事项使用前应该振摇，使用后剩余的得普利麻不管多少，均应该丢弃。必须保持得普利麻输注装置无菌（参见其它注意事项）对驾驶或操作机器能力的影响病人应该得到忠告，即全身麻醉后的某些时候，技能性工作如驾驶和操作机器的能力可能受影响。【孕妇及哺乳期妇女用药】妊娠妊娠期间不应使用丙泊酚，但在妊娠的前三个月终止妊娠时，已经有使用得普利麻的经验。产科得普利麻能透过胎盘，并可能与新生儿的抑郁有关，该药不应用于产科麻醉。哺乳正在哺乳的母亲使用得普利麻后，对新生儿的安全性尚未被肯定。【儿童用药】 请参见【用法用量】部分【老年患者用药】请参见【用法用量】部分【药物相互作用】得普利麻已经用于脊髓及硬膜外麻醉，并与常用的术前用药、神经－肌肉阻滞 剂、吸入麻醉剂及镇痛剂合用，未发生任何药理学上的配伍禁忌。当局部麻醉中须辅以全麻时，可减少得普利麻用量。配伍禁忌除了可与5%葡萄糖液在PVC输液袋或玻璃输液瓶中混合、与利多卡因及阿芬太尼注射液在塑料针筒中混合外，得普利麻在使用前不应与任何注射液混合。神经－肌肉阻滞剂，阿曲库铵及美维库铵在变红前不能与得普利麻经同一静脉输液通道给药。【药物过量】意外性用药过量，可能引起心脏和呼吸抑制。应该使用氧气进行人工通气来处理呼吸抑制。对于心血管抑制的治疗，要求把病人的头部放低，如果抑制严
